TOTAL COMPENSATION PACKAGE: The salary for this position is $73,000-$85,000, depending on experience. CAN offers a generous benefits package available on the first day of the month after your date of hire that includes: 

  • Medical insurance through Kaiser or Sutter is 80-95% paid by the employer for employee premiums plus 50% paid by the employer for dependent coverage

Summary Of Position:

Reporting to SC|SU/HA Together’s Senior Management, the Program Manager is responsible for managing the planning and implementation of the Resource Connection Network program which includes implementation of grants, staff management, budget management, and coordination of all related grants and projects within the RCN program. The Resource Connection Network (RCN) supports closed-loop referrals across organizations and sectors to better support community members to navigate and access resources and is an integral piece of the Community Information Exchange (CIE) Model. Through integration of data systems, we create a more cohesive and manageable ecosystem of cross-sector services to support our community. The Program Manager works collaboratively with other SC|SU & HA Together backbone team members in addition to community partners,  initiatives,  and vendors, and is able to work independently to achieve program-related goals on time and within budget.

Essential Duties And Responsibilities:

  • Manage the planning, implementation, and evaluation of the Resource Connection Network (RCN) to ensure program goals, deliverables, and grant requirements are met.
  • Oversee RCN data governance, compliance, and integration efforts, ensuring systems are equity-focused, community-driven, and legally compliant.
  • Lead the research and development of platform modification and integration priorities, gathering input and insight from stakeholders and experts as appropriate.
  • Develop and present agendas, reports, policies, and procedures to support program outcomes, RCN user success, and provide updates to leadership, advisory groups, staff, and other stakeholders as needed.
  • Stay abreast of information about local, state, and national policies, best practices, and trends to lead community-centered Data and Technology Policy development.
  • Work closely with the Communications Manager and support staff to plan and implement timely and ongoing communications related to the Resource Connection Network.
  • Supervise and support staff, interns, and volunteers through coaching, mentoring, and professional development in alignment with the mission, vision, and culture of Sonoma CAN, Sonoma Connect, and Health Action Together.
  • Lead the onboarding, training, and engagement of new and existing partners to the CIE-model network and RCN platform, including outreach, meetings, workflow design, stakeholder presentations, and ongoing relationship management to ensure alignment and program success.
  • Collaborate effectively and respectfully with vendors and consultants to support platform development, troubleshoot issues, and improve user experience. Oversee requests for Quotes, Scopes of Work, Change Orders, and Help Tickets.
  • Positively represent Sonoma CAN in community meetings, partnerships, and presentations to build alignment and foster collaboration.
  • Monitor and report program outcomes using quantitative and qualitative data, making recommendations for improvements.
  • Project manage interoperability discovery process between Resource Connection Network and other record management systems leveraged by Community Based Organizations, Health Centers and Hospitals, as well as Education, Tribal, and Government Partners.

Requirements

Required Skills/Abilities:

  • Understanding of program and grant lifecycles, including budgeting, reporting, and compliance.
  • Knowledge of data governance, privacy regulations (ex. HIPAA/PHI), and experience with care coordination, Electronic Medical Record//Electronic Health Record, or other client record management systems.
  • Ability to work effectively in multicultural environments and remain calm under pressure.
  • Spanish language proficiency preferred; biliterate skills may receive premium pay.
  • Valid California driver’s license and access to a properly insured vehicle.
  • Ability to pass a background check prior to employment.

Required Education And Experience

  • At least three years of experience in a community-based and/or non-profit health or human services setting.
  • Minimum one year experience with successful implementation and coordination of a grant funded program.

Preferred Education And Experience:

  • Bachelor’s Degree in non-profit, public health, social science, education, health, public policy, public administration, or related discipline.

Two years of experience in public health, healthcare, social care, or related area.

  • Project management methodology education or certification.
  • Experience developing and delivering training programs in a comparable setting.
  • Experience successfully implementing grants – proposals, budgets, accountability, reporting.
  • Experience building relationships with managing subcontractors.
  • Experience implementing Community Information Exchange-model networks and knowledge of CA Data Information Exchange Framework, CalAIM, SDOH, HIPAA and PHI data sharing regulations
  • Experience working with care coordination, EMR/EHR, or other IT based information or client management systems
  • Lived experience by serving or being served by community health centers, understanding of race equity, especially as it pertains to community wellbeing and needs, and commitment to anti-racism throughout the  work and in the development of
  • Informatics, data sharing/governance, and/or technology platform related experience and/or advanced degree.

Work Environment:

The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. This job operates in a professional hybrid (in person & virtual) office environment. The noise level in the work environment is usually moderate. Attendance at in-person meetings in Sonoma County is required at least twice a week.
