Job Duties

  • Oversee the admissions and discharges flow of the hospital, maintaining awareness of bed status and barriers to discharge.
  • Serve as an active member of the interdisciplinary team providing a social work perspective.
  • Plan, organize and implement a full range of clinical services to patients and their families including initial and ongoing assessment, individual, family and group counseling.
  • Educational needs, assessment and provision of education as indicated.
  • Coordination of discharge planning with the Community Service Board (CSB).

Minimum Qualifications

  • Experience as a social worker in an inpatient healthcare setting working directly with patients.
  • Master's degree in social work from an accredited college/university required.
  • Knowledge of admission and discharge processes.
  • This position will also include:
  • Knowledge of population specific factors in assessment and treatment.
  • Knowledge of theories of human behavior and development.
  • Skills and ability in providing individual, family and group counseling.
  • Knowledge of community resources.
  • Advance interviewing skills.
  • Knowledge of Social Work Code of Ethics.
  • Ability to make independent work decisions.
  • Ability to independently maintain clinical records on assigned cases in accordance with hospital policies and governing body regulations.
  • Knowledge of SVMHI admissions process.
  • Successfully complete and maintain throughout employment CPR, Therapeutic Options (TO) training, and other required training as required by the facility.
