As a Hospice Nurse Practitioner, you are responsible for conducting in person face-to-face patient visits as required by Medicare regulations, gathering and documenting clinical data, and supporting the Hospice Medical Director in determining initial and ongoing eligibility for hospice care. This role collaborates with the interdisciplinary team, ensures compliance with regulatory guidelines, and contributes to the overall quality of hospice services

This position is paid on a per-visit basis at a rate of $150 – $250 per completed visit, depending on experience and geographic location. Rates are subject to periodic review and may be adjusted based on organizational and market considerations.

  • As a Hospice Nurse Practitioner, you will:  
  • Clinical Assessments & Encounters – Conduct in-person face-to-face visits with hospice patients at required benefit periods, performing comprehensive physical, functional, and psychosocial assessments.
  • Documentation & Compliance – Accurately document patient encounters in the EMR, ensuring timely submission that meets Medicare, CMS, and agency regulatory requirements.
  • Collaboration & Communication – Share findings with the Hospice Medical Director and interdisciplinary team to support care coordination and eligibility decisions. 
  • Patient & Family Support – Provide compassionate, professional care during visits, serving as a resource and advocate for patients and their families. 
  • Other Duties as Assigned – The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive. 

This Job Also Requires:

  • Current, unrestricted Nurse Practitioner license in the state of Minnesota. 
  • Current National Board Certification (e.g., AANP or ANCC).
  • Valid DEA license (if required by state).
  • Minimum 1 year of NP experience; hospice experience preferred.
  • Strong clinical assessment and documentation skills. 
  • Reliable transportation, valid driver's license, and proof of auto insurance.
  • Ability to work independently, manage time efficiently, and maintain compliance.
  • Ability to pass state mandated background checks.
  • Physical capability to perform all essential job functions
  • Ability to read, write, and speak English to ensure effective communication with team members, clients, and visitors.
