About The Team

Our pharmacists are recognised as essential members of all clinical teams, actively engaging in a unit and team-based care model involving ward rounds and collaborating closely with the multidisciplinary team to achieve excellent patient care outcomes. The clinical pharmacy service extends beyond the traditional hospital pharmacy setting, operating across 7 days and extended hours, to ensure consistent patient care regardless of when they are admitted. Pharmacy programs span across all aspects of the patient journey, from acute admission through to ambulatory and post discharge innovation programs, including pharmacist-led and pharmacist integrated ambulatory clinics.

There are vast opportunities to engage in Advanced Scopes of Practice. This includes undertaking credentialing programs and leading clinical activities such as Partnered Pharmacist Medicine Charting, Therapeutic Drug Monitoring, Nicotine Replacement Therapy, and Analgesia de-escalation, to name a few.

The service has 3 key priorities:

  • Practice
  • Education
  • Research

As part of these priorities, the department holds a breadth of clinical stewardship and governance portfolios to ensure high quality and safety standards at all times. The department also has advanced Education programs with Foundation Residency and Advanced Training Residency to support career progression, including opportunities to rotate to external hospitals. Many of our innovative clinical programs have been adopted nationally and are linked with novel educational initiatives.
