General Summary: The Rehabilitation Specialist will educate members to function within their community. Treatment can address relationships with family, friends, and co-workers and often helps members work toward living independently. This position will work closely with a team of professionals both within and outside the organization to achieve successful social integration for the member.
JOB RESPONSIBILITIES:
• With the assistance of an integrated care team, will develop treatment plans that help the member learn skill sets needed for independent living. Will monitor patients' progress, help them achieve their social integration goals, and intervene in crisis situations.
• Rehabilitation Specialists may provide assistance with job training, housing needs, and family support issues.
• Participates in the assessment, evaluation and planning of patient care; revising as necessary.
• Provides community and home-based services to enhance and achieve treatment goals.
• Participates in Interdisciplinary Team meetings, clinical staffing’s, case conferences and other meetings as appropriate. Consults with treatment staff to coordinate effectiveness of services contained in the member’s individual service plans and make adjustments where needed.
• Maintains appropriate documentation (charting) and documents accurately and timely in accordance with agency and funding source requirements.
• Provides member advocacy and education as needed, to ensure delivery of required services.
• Preserves the human dignity and the confidentiality of each member.
• Responsible for following any policies, procedures, and controls established by the organization, the HIPAA Privacy Officer, and/or the HIPAA Security Officer regarding access to, protection of, and the use of the PHI.
• Performs other duties as assigned or necessary as they relate to the nature of the position.
QUALIFICATIONS:
• Education – Bachelor’s degree in a Behavioral Health related field. Or equivalent experience.
• Experience – One (1) year of Behavioral Health experience. Experience in medical or behavioral health clinic setting.
REGULATORY:
• Minimum 18 years of age.
• DPS Level I fingerprint clearance (must possess upon hire and maintain throughout employment).
• CPR, First Aid, AED certification, if required (must possess upon hire and maintain throughout employment).
• Current, valid Arizona Driver’s License and 39-month Motor Vehicle Report and proof of vehicle registration liability insurance to meet insurance requirements, if required.
• Initial current negative TB test result, if required (Employer provides).
