Job Summary: The Assistant Director of Nursing for Assisted Living and Memory Care (ADON-AL/MC) is responsible for the overall leadership, clinical oversight, and operational management of the Assisted Living (AL) and Memory Care (MC) neighborhoods within a vibrant CCRC. This individual ensures delivery of exceptional, person-centered care that promotes dignity, wellness, and engagement for all residents. The Director fosters a collaborative environment with residents, families, and interdisciplinary teams to support quality outcomes and regulatory compliance.

- Serve as the primary clinical contact for residents and families in AL and MC.
- Facilitate in resident/family care plan meetings and address concerns with empathy and professionalism. Identify and communicate any significant changes to the POA.
- Promote programs and services that enhance quality of life, especially for those living with cognitive impairment.
- Develop and monitor quality assurance initiatives and key performance indicators.
- Maintain accurate documentation and audit resident records for compliance and quality.
- Ensure effective infection control, medication management, and incident reporting procedures.
- Oversee delivery of resident-centered care in accordance with individualized care plans and clinical best practices.
- Ensure regulatory compliance with Illinois Department of Public Health (IDPH) and other applicable regulations.
- Conduct assessments, review medical records, and coordinate care transitions with other levels of the care continuum.
- Respond to changes in condition and implement appropriate interventions.
- Staff oversight and supervision.
- Partner with the Scheduler to monitor staffing schedules to ensure appropriate coverage and cost control.
- Lead regular team meetings and participate in leadership and QAPI committees as needed.
- Develop strong partnership with the Medical Director to best maintain overall physical and emotional health.
- Approve training protocol and require signatures for completion to hold staff to a higher standard of care.
- Comply with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all Plymouth Place policies and procedures.
- Maintain regular and punctual attendance at work and meetings.
- Attends in-service and education programs.

Requirements
Job Requirements:
- Licensed Registered Nurse (RN) in the State of Illinois.
- Minimum of 3 years of supervisory or leadership experience in Assisted Living, Memory Care, or long-term care.
- Experience within a CCRC or multi-level care setting required.
- Dementia care certification (i.e., CDP) required.
Knowledge, Skills And Abilities Required:
- Experience in memory care and assisted living recommended.
- Understanding of Point Click Care or similar systems.
- Knowledge of monitoring medication administration.
- Strong knowledge of IDPH regulations for AL and MC
- Skilled in dementia related care and programming.
- Proven ability to lead teams, resolve conflicts, and manage change.
- Strong commitment to customer service. • Excellent communication and interpersonal skills.
- Ability to work independently and multitask.
- Proficiency with Microsoft Office Suite or related software.
- Strong organizational skills and attention to detail
. • Ability to read, write, and speak English sufficiently to perform job duties and interact with residents, families, and employees.
ENVIRONMENTAL AND PHYSICAL ACTIVITY REQUIREMENTS: The physical activities of this position constantly require effective talking, hearing, sitting, walking, and standing functions. The position also often requires exerting up to 50 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Must be able to perform activities with close visual acuity, such as reading a computer screen and reading print material. The functions of this position are conducted in a temperature-controlled environment.
OSHA Exposure Category: Category I – Position includes tasks that involve exposure to blood, body fluids, tissues; would be required to perform Category I tasks.
INTELLECTUAL/EMOTIONAL REQUIREMENTS: The occupant of this position must be able to perform the responsibilities of the position well under pressure. The position requires adaptability to performing a variety of duties, often changing from one task to another without loss of efficiency or composure. This position requires the ability to deal with and find resolutions to problems and maintain a productive working relationship with employees and may be interrupted frequently to meet the needs and requests of others. It also requires adaptability to fluctuate between priorities daily while being responsive to needs as they arise.
