AccordCare Home Health, an accredited home health agency, is seeking an experienced Care Coordinator to join our Maitland, FL office. This pivotal in-office role ensures our team and patients are supported through efficient scheduling and coordination, specifically for Private Duty Home Care services.
🔹 Key Responsibilities
- Schedule and coordinate all field staff visits in accordance with the Plan of Care for private duty clients
- Build and maintain strong relationships with staff, sales teams, and referral sources to ensure efficient service coordination
- Work closely with the Clinical Manager to manage patient schedules
- Maintain accurate records in the computer system with schedule changes and visit authorizations
- Communicate with Intake, referral sources, sales staff, and team members to facilitate patient care
- Collaborate with the Billing Department as needed
- Facilitate payroll and participate in on-call rotation
- Address and resolve patient/family scheduling concerns
✅ Qualifications
- Minimum 1 year of Private Duty Home Care scheduling experience required
- Strong written and verbal communication skills
- Excellent customer service skills
- Ability to work effectively in a fast-paced, office-based environment
