iCare is seeking a Nurse Practitioner who will provide advanced level case management and care coordination for Family Care Partnership members in collaboration with the Interdisciplinary Team (IDT).The Family Care Partnership Nurse Practitioner works together with the IDT staff to provide clinical expertise to members, understanding the Family Care model and working within those parameters, working collaboratively with IDT, Partnership Associates and other iCare departments. The Nurse Practitioner provides assessment for members as needed including members who are experiencing acute care needs, collaborating with the IDT on findings and plan development. The Nurse Practitioner assists IDT and members with oversight, education and guidance around health promotion and maintenance activities. The Nurse Practitioner collaborates on discharge planning, LOS, and subacute staffing, providing education and support to staff on PA/UM process and procedure.
Required Qualifications
Master’s Degree in Nursing.
• Possession of a licensure or eligibility for licensure in the State of Wisconsin as a Registered Nurse, Advance Practice Nurse Prescriber (APNP) licensure, and national NP certification in geriatrics, adult or family practice.
• Two years of previous experience working with persons in any of all of the target member populations: (frail elderly, physically disabled, developmentally disabled) in a health care setting.
• Must be able to travel to any location in Milwaukee Co. with some travel possible in other counties within Partnership contracted counties.
• Strong assessment and care planning skills required along with the ability to provide complex care coordination.
• Ability to demonstrate flexibility, set priorities with daily demands and long-term work assignments and projects.
• Strong interpersonal skills and ability to effectively interact with providers, members and co-workers from a variety of different backgrounds and experiences in a professional and courteous manner.
- Preferred Qualifications:
- • Previous home health, managed care or case management experience.
- • Previous experience working with persons in any of all of the target member populations: (frail elderly, physically disabled, developmentally disabled) in a health care setting.
- • General knowledge of Medicaid and Medicare benefits

Driver's License, Transportation, Insurance This role is a part of Humana's Driver Safety program and therefore requires and individual to have:

Tuberculosis (TB) screening program
- This role is considered patient facing and is part of Humana at Home's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$115,200 - $158,400 per yearThis job is eligible for a bonus incentive plan.
