Purpose of the Position
The Case Worker as part of Central Queensland Indigenous Development Ltd (CQID) organisation supports the achievement of CQID's strategic goals of best practice and innovative alignment with progressive government reforms by effectively leading the professional practice within a specific portfolio of programs and services across the CQID service footprint.
The Next Step Plus Senior Case Manager leads a therapeutic collaborative approach to engaging and supporting young people aged 15 years up to 25 years who are under or have transitioned from the Guardianship of the Department of child Safety and who are on a journey to achieve their independence focused outcomes.
In addition to taking lead on a managing complex care and support cases, this rile will also be responsible for providing case specific mentoring, coaching and practice directive to case management staff partnering with young people towards their transition to adulthood and independence focussed outcomes.
This role may be required at times to travel to relevant locations for service delivery or program/practice development purposes.
Responsibilities And Duties
- Support and walk alongside the young person to enhance their capability to adapt and connect with goal-oriented supports and interventions that are aligned to independence focussed outcomes.
- Lead the facilitation of coordinated care integrating organisation and external service supports that are aligned to agreed goals.
- Support the development and maintenance of relationships with significant family members, friends, community organisations and government agencies to promote lasting connections and positive outcomes for young people.
- Work collaboratively with Child Safety, government and community stakeholders supporting young people in care (15+) in the development of a transition plan and gaining the skills, knowledge and networks they need including identifying a natural mentor.
- Facilitate and support opportunities that enable young people to maintain and strengthen their cultural identity through meaningful cultural connections.
- Participate with the wider leadership team in Next Step Plus & Extended Post Care Support Program to drive routine reviews and continuous improvement activities in line with the program objectives and practice manual.
- Development by providing relevant advice to the Manager.
- In consultation with the Program Manager, lead the Case specific practices:
- Lead regular and routine case/care reviews with case management staff to ensure young people’s care is being supported appropriately and there is progress towards goals that are representative and reflective of Young People’s voices and choices.
- Provide leadership and case specific directions to staff on service delivery to young people, allocation of internal resources or allocated funding to service delivery and support staff.
- Lead place based key stakeholder engagement and partnership activities that benefit and advance outcomes for young people.
- Complete all delegated record keeping requirements for compliance, accountability, reporting, evaluation purposes and contribute to and participate in continuous improvement activities.
- Maintain and model a practice of comprehensive professional case notes, reports, client files and records, including data collection according to program and statutory requirements, and ensure timelines for completion are adequately met.
- Model the mission and values of CQID at all times, model the code of conduct, policies and procedures and relevant government legislation and standards where relevant.
- Work always to advance the reputation of CQID and undertake additional relevant duties as required.
- Participate and contribute to the organisation’s compliance obligations under the ISO 9001:2015 Quality Management System and Human Services Quality Standards (HSQS).
- At all times work within CQID’s Workplace Health & Safety Policies and Procedures.
- Other responsibilities as directed by the Manager and/or General Manager.
Qualifications And Experience
To be successful in this role you will need to demonstrate capability in the following areas:
Essential
- Hold a current C Class Queensland Driver’s Licence, current Working with Children Check, 'Blue Card'
- Holds or is willing to obtain a First Aid and CPR Certificate
- A working knowledge and demonstrated experience of Indigenous communities and cultures and the historical and contemporary issues relevant to the needs of Aboriginal and Torres Strait Islander clients, and proven capacity to communicate appropriately
- A working knowledge child protection issues, legislation and requirements and relevant issues specific to Indigenous children and their families
- Demonstrated experience in negotiating and partnering with diverse agencies and organisations that enable young people to achieve independence eg services, schools, social welfare, Court, Police, Legal and health agencies
- Demonstrated experience in person centered case management and/or coaching/mentoring service delivery and support for young people with complex needs
- High level interpersonal skills including a commitment to quality service/support provision, and the capacity to exercise initiative, responsiveness, discretion and maintain a high level of confidentiality
Desirable
- Relevant Tertiary qualifications within the community services sector and/or relevant experience
- Participation in Therapeutic Crisis Intervention training
- High level written communication skills for the creation of reports, correspondence and other material
