Role Overview:
The Director of Clinical Transformation serves as a critical component for managing the TEAM model with our partner Hackensack Memorial Health in New Jersey. This Director will oversee the episode performance for the 9 hospitals of the HMH system that will participate in TEAM. This individual will be the in-market representative expert that regularly engages with stakeholders to produce continued performance and ensure clinical and financial success. The Director of Clinical Transformation will oversee a team of Episode Coordinators to ensure optimized patient outcomes.
As a Director of Clinical Transformation you will:
- Lead the Hackensack Meridian Health TEAM episodes to ensure success in performance
- Facilitate co-design of episode pathways with Navvis physician support
- Support adherence to workflows and refine pathways as indicated
- Review clinical and operational reports to identify gaps in performance and develop strategies to address
- Manage Care Episode Coordinators through management of caseload and success
- Facilitate hospital and system report outs to ensure awareness of TEAM performance
- Serve as an In-market subject matter expert on TEAM to system stakeholders
- Prepare high quality and compelling presentations for delivery to clients and participate in subsequent action planning related to gaps and opportunities
- Coordinators being managed will cover these locations:
- o Hackensack University Medical Center
- o Palisades Medical Center (Safety Net)
- o JFK University Medical Center
- o Raritan Bay Medical Center
- o Jersey Shore University Medical Center
- o Bayshore Medical Center
- o Riverview Medical Center
o Ocean University Medical Center
o Southern Ocean Medical Center

Requirements
We are excited about you if you have these things:
- RN or NP required.
- 5-7 years working to improve Care Episode performance for a Health System, Payer, or Physician Organization in these areas:
- Superior writing and presentation skills
- Strong critical thinking, problem identification, resolution skills
- BPCIA and CJR Models
- Advanced Alternative Payment Models such as MSSP, NGACO, ACO Reach, Direct Contracting
- Medicare Advantage Health Plans
- Population-based Improvement Strategies
