What are the job responsibilities?
Reporting to the VP of Business Development, the Clinical Liaison (RN) serves as a bridge between referral sources, patients, families, and the hospice care team. This role ensures timely evaluation of potential patients, provides clinical education to referral partners, and facilitates smooth transitions into hospice services. By blending strong clinical expertise with compassionate communication, the Clinical Liaison plays a critical role in both growing Cadre’s presence in the community and ensuring patients receive appropriate, high-quality end-of-life care.
Key Responsibilities Include:
- Receive and respond to referrals from hospitals, home health, physicians, case managers, skilled nursing facilities, and other healthcare professionals
- Assess patient eligibility for hospice services by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families
- Educate patients and families on what hospice care includes, how it works, and why it matters—providing clarity, reassurance, and compassion
- Build and maintain strong relationships with referral partners and identify new outreach opportunities to increase awareness of hospice services
What do you need?
- Current RN license in the state of practice required.
- Minimum of two years of referral development, marketing or sales within healthcare plus at least two years of clinical experience in one of the following: hospice, home health, liaison, clinical coordination, discharge planning, case management or RN Educator.
- Clean driving record, and valid driver’s license and auto insurance
