- The initial compensation for this position ranges from $21.00 - $25.83 per hour.
- Salary is dependent on commensurate experience above the minimum qualifications for the role and internal equity considerations.
- The salary may also vary if you reside in a different location than the location posted.

Who We Serve
Pacific Clinics serves children, transitional age youth, families, adults, and older adults. We offer a full range of mental and behavioral health services, foster care and social services, housing, continuing adult education and early childhood education programs to Medi-Cal eligible individuals and families throughout Alameda, Contra Costa, Fresno, Kings, Los Angeles, Madera, Orange, Placer, Riverside, Sacramento, San Bernardino, San Francisco, Santa Clara, Solano, Stanislaus, Stockton, Tulare and Ventura Counties.

Program Description: Works within the vision, mission, and philosophy of the agency and under direct supervision, supports with resources, develops curriculum, facilitates parent education activities, delivers presentations and workshops, and coaches staff that support the needs of youth and parents receiving agency services. Inspires and secures community engagement, develops partnerships with outside agencies to increase strength based opportunities to enhance or create innovative youth and parenting activities. Maintains linkages and community relationships via outreach and works with the department to increase customer's positive engagement in the community and accessing its resources. Supports customer and program needs as needed.

Job Summary

Maintains linkages and community relationships and works with the department to increase customer’s positive engagement in the community and accessing its resources.
Responsibilities And Duties
- Develops and maintains linkages and relationships with community resources that support the needs of the department (e.g., foundations, churches, neighborhood groups, schools, and grassroots organizations).
- Works alongside programs to support the efficiency of daily operations (e.g. outcome measures completion, ensure programs have appropriate supplies, outreach, referrals, overall support to program etc.).
- Facilitates regional work groups that support comprehensive identification and linkage to community resources to meet family needs.
- Identifies, solicits, and obtains targeted resources that meet the individualized needs of families.
- Develops and maintains databases and resource guides with agency and community resources to support customer access to available services.
- Develops and implements educational and awareness activities aimed at promoting community involvement and resource access.
- Plans, attends, facilitates, and publicizes special events and meetings that support youth and families in experiencing connection to their community.
- Assists in the development, implementation, and evaluation of projects.
- Collaborates with community partners to identify and provide peer-navigator training and support to community members.
- Trains, mentors, and coaches customers on community resource access and utilization.

Minimum Education And/Or Experience
- High School Diploma or GED required.
- Bachelor of Arts/Bachelor of Science (B.A./B.S.) preferred.
- One (1) to three (3) years applicable experience initiating, implementing, and coordinating activities that interface with the general public; or
- One (1) to three (3) years experience working in a community non-profit setting or social services setting.
Other Specific Requirements
- Ability to represent the agency and its mission to the general community.
- Excellent communication and presentation skills.
- Proficient in Microsoft programs and navigating technology.
- Knowledge of community resources.
Preferred Qualifications
- Bilingual / bicultural (English-Spanish or other) skills.
- Project Management experience.
- Prior experience working in an education or social service field.
