The Role:
The Community Supports Coordinator, AWC manages the full onboarding process for caregivers and members within the Agency with Choice service line. This role ensures a smooth and timely transition from referral to admission, maintaining compliance and high-quality service delivery. By serving as the main point of contact for members and caregivers and collaborating with internal teams, the coordinator supports program growth and a seamless service experience.
Primary Responsibilities
- Lead the end-to-end onboarding process for caregivers and members, ensuring all required documentation and compliance standards are met.
- Act as the primary contact for new members and caregivers, providing timely updates and clear communication throughout onboarding.
- Maintain accurate, real-time caregiver and member records in Salesforce and other tracking systems, including status updates and key metrics.
- Educate and support caregivers on timekeeping policies, recording procedures, and program requirements.
- Respond to inquiries from members and caregivers via phone, text, chat, or email, delivering one-call resolutions and excellent customer service.
- Communicate with health plans and case managers to meet closed-loop referral and care integration requirements.
- Support the Community Supports team with ad hoc tasks, meetings, and special projects as needed.
This is a hybrid position, coming into the El Segundo office 1x per month.
WHAT YOU BRING TO THE TABLE:
Skills
- Strong communication and relationship-building skills with members, caregivers, and internal teams
- Excellent time management and organizational ability to handle multiple cases simultaneously
- Attention to detail and accuracy in processing applications and maintaining records
- Problem-solving and decision-making skills to quickly address onboarding challenges
- Proficiency in Microsoft Office and case management systems (e.g., Salesforce)
- Positive, solution-oriented mindset and ability to meet time-sensitive deadlines
- Customer service or service-oriented experience (minimum 1 year preferred)
- Bilingual Spanish skills preferred
- Ability to work independently and adapt to changing priorities
- Flexibility to travel occasionally for in-person meetings, events, or conferences
