The Role
The Care Manager based in North Strathfield is a part of the aged care team and is integral to the delivery of high-quality support to our clients. With an extensive knowledge of the Home Care Packages Program, the Care Manager role is responsible for supporting Focus Care's aged care clients by providing client focused care management.
The Care Manager will use a collaborative approach to assist clients to access available and relevant resources necessary to attain their identified goals within the funding and quality regulations.
The Care Manager will further be directly responsible for budget management, lead management, client screening, client assessments, risk identification and mitigation strategies, planning and implementing support strategies, monitoring and evaluating supports and managing transition to other forms of support and care, relevant to the clients' changing needs.

In Addition, You Will Have:
- Extensive experience as a HCP Care Manager, with knowledge and understanding of the Aged Care Quality Standards (current and strengthened).
- Excellent verbal and written communication skills with experience in complex care management.
- Be enthused by the reform process, and the opportunities it presents.
- High level problem solving and critical thinking skills.
- Nursing qualification with AHPRA Registered Nurse or Enrolled Nurse Registration.
- A valid drivers licence.
Key Accountabilities
You Will Be Responsible For:
- Program Management – Applying Focus Care's processes and procedures to ensure best practice Community Aged Care services that exceed the Aged Care Quality Standards.
- Care Management – Active care management of HCP (Support at Home from 1 November), including operational lead management, client screening, assessments, risk identification and mitigation strategies, planning and implementing support strategies, monitoring and evaluating supports and managing transition to other forms of support and care, relevant to the clients changing needs.
- Business Development – Proactive lead generation and management, ensuring sustainable business growth is achieved across the aged care portfolio.
- Business Compliance/Change Readiness – Working with the management team to monitor legislation, standards and program guidelines and the reform process, to ensure knowledge is current, the organisation is compliant and ready for Support at Home commencement.
- Process Improvement – Ensure best practice care planning, assessment and review processes and develop and implement training to enable a depth of geriatric literacy across the aged care team.
