Job Summary
The Justice Involved Health Navigation Specialist will provide connections to resources, referrals and navigation services to Gunnison County residents who are involved in the criminal justice system and/or child welfare system. Resources/referrals will be related to physical health, behavioral health, and basic needs. Support will also be given to help mothers be successful in completing their probation/family plans. The Navigator will communicate directly with the client and/or their legal representative/family to determine what level of assistance they require. The Navigator will assist the client to navigate the systems of care and resources available to the client to increase access to care and services. Light case management is part of this position. This is a pilot program with grant funding for 12-15 months.
This position is currently supported by grant funding, which allows us to advance the county's strategic initiatives. Employment beyond the current grant period is dependent on funding availability.
Pay Rate
- The hourly range is from $27.84 to $31.69, depending on experience, plus partial benefits.
- This position is 10 hours per week.
- *The County pays monthly on the last week day of the month.*

Requirements
- Must possess a valid Colorado driver’s license.
- Must complete ICS 100 and ICS 700 within the first year of employment.
Qualifications/Education
- Post-secondary education in criminal justice, health care, social sciences or related field.
- Bachelor’s degree preferred. At least 1 year of experience in health or human services field preferred.
- Any equivalent combination of education and experience that the County deems appropriate.
