The Role:

The Outreach Coordinator serves as the first point of contact for families referred by Regional Centers, ensuring a smooth and supportive start to services. This role coordinates all aspects of new consumer intake, maintains accurate records, and provides timely updates to families, providers, and Regional Center staff. By fostering strong relationships and collaborating with internal teams, the Outreach Coordinator ensures an efficient, high-quality experience for every new consumer.

Primary Responsibilities

  • Serve as the initial contact for new families, responding to inquiries and guiding them through service onboarding.
  • Coordinate services for new consumers referred by Regional Centers, ensuring referrals are addressed the same business day.
  • Conduct and document intake processes, assign cases, and communicate consumer needs to Operations and Community Partnerships teams.
  • Retrieve provider applications, assist families and providers through the process, and follow up to ensure timely completion.
  • Track referral progress and maintain accurate records and metrics in Salesforce and related systems.
  • Maintain open, professional communication with families, providers, and Regional Center staff to resolve questions and issues quickly.
  • Support team initiatives and complete other duties as assigned to enhance service quality and operational efficiency.

This is a hybrid position, coming into the Roseville office 1x per week.

  • Strong interpersonal and communication skills for families, providers, and referral partners
  • Excellent time management and organization to handle multiple referrals and follow-ups
  • Problem-solving ability and resourcefulness in resolving intake or application issues
  • Positive, energetic, and self-motivated approach to achieving service goals
  • Attention to detail and discretion when handling confidential information
  • Proficiency with Salesforce and related case management or communication tools
  • 1–2 years of customer service or client support experience
  • Reliable attendance and ability to work in a hybrid setting (on-site as scheduled or requested)
