📣 LIFE (Living Independence for the Elderly) is a Medicare/Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility. Job Description

• Document interdisciplinary progress notes that provide a comprehensive reflection of resident care.
• Administer medications and treatments in strict adherence to physician orders. Maintain accurate and up-to-date drug and narcotic records for each shift.
• Report any discrepancies to the center manager and follow proper procedures for drug and narcotic disposal.
• Collaborate with the care plan team to assess and address residents' physical, social, nutritional, spiritual, and activity-related needs.
• Perform home visits with participants as needed.

Qualifications

• One year experience working with a frail or elderly population.
• Active Pennsylvania Licensed Practical Nurse (LPN) License.
• Current CPR and First Aid certification.
• Valid PA Driver's License.
• Compassion, patience, and genuine desire to make a positive impact on the lives of seniors.
