Job Description

Act as supervisor for Personal Care neighborhood. Some shifts will include working on the med cart.

• Develops and maintains methods for clear communication within the department as well as communication between department and physicians, Residents, and families. Participates in weekly Care Navigation meetings.
• Makes daily rounds and audits, including shift report to ensure proper care, documentation, and order transcription to maintain high standards for this department.
• Provides education and mentoring to department staff working on the floor.
• Covers on-call for call-offs through rotating schedule as needed.
• Performs other duties, as assigned.

Qualifications

Current LPN license in the state of PA & CPR certification
