As a Care Advisor in our South West Sydney region, you’ll ensure clients’ needs and preferences are met through effective and sustainable case management. You will play a critical role in aligning services with Anglicare’s strategy, quality standards, and values, ensuring that our clients receive the highest level of care and support.

This is a permanent, full-time position (Monday to Friday) based in Minto or Oran Park, with regular client visits in the South West Sydney region and travel to Wollongong for team meetings and training.

Your Opportunity

With care and professionalism, you will:

  • Provide comprehensive case management, tailoring services to meet clients’ physical, clinical, emotional, spiritual, and social needs
  • Assess and identify client needs, coordinating services and products (including assistive technology) within budget constraints
  • Ensure compliance with policies, the Care Packages Program and legislative requirements, maintaining accurate documentation
  • Foster strong relationships with teams, health professionals, clients, families, and stakeholders to enhance Anglicare’s profile
  • Assist clients in managing their care budgets, ensuring correct allocation and accounting
  • Support team members in understanding and implementing Anglicare’s strategy, policies, and values, while monitoring performance and gathering client feedback

What You’Ll Bring

You’re dedicated, compassionate and thrive in a role where you can make a meaningful difference in the lives of older people.

You Will Need:

  • Certificate IV in Aged Care, Community Services, or equivalent
  • Previous experience in home care, community services or aged care
  • Intermediate computer skills, including MS Office proficiency
  • Current NSW driver’s licence
  • A commitment to Anglicare’s vision, mission and values

Ideally, You Will Also Have:

  • Diploma in Case Management (desirable)
  • Experience supporting clients under the Aged Care Quality Standards
