Summary:

This Social Worker will provide services to children and families at our High-Risk Asthma Clinic. Responsibilities include providing case management services, conducting psychosocial assessments, providing social and emotional support to patients and families, assisting families with insurance, scheduling transportation, and assessing the family's need for referrals/linkage to resources. Will work with a multi-disciplinary team of healthcare professionals to uphold the Cook Children's promise: Knowing every child's life is sacred, we promise to improve the well-being of every child in our care and our communities.

Hours: 20 hours per week; 8-hour days (normally Thurs/Fri) and one 1/2 day. Position will work on a part-time basis Mon - Fri, but days may vary week to week depending on clinic needs.

Education And Experience:

• MSW or MSSW from an accredited school.
• Experience working with children and families (preferred)
• Previous experience in a hospital setting (preferred).
• Prior asthma/pulmonology experience (preferred).

Licensure And Certification Required:

• Current LMSW Licensure with the state of Texas.
• Basis Life Support (BLS) Certification required within 30 days of employment.
