The Manager of Intake Referral for Home Health and Hospice will be accountable for ensuring high quality, efficient customer service for patient care from all External, AMG referrals, and collaborate with other product lines, Infusion pharmacy, PAN SNFs, full risk referral from population health, and Advocate/Aurora inpatient facilities for safe transitions into the community.

Major Responsibilities:

  • This leader exhibits and mentors a thorough understanding and commitment to the principles of customer service as a foundation for this position. Responsible for the management of hospice and HH referrals to meet goals and objectives in accordance with national and system standards, policies, and regulatory requirements. Responsible for implementing systemwide quality services and business development strategies.  
  • Responsible for meeting and exceeding budgeted revenue growth through efficient employment and management practices. Collaborates with other departments within the post-acute and inpatient hospital system. 
  • Establishes and monitors standard workflows to provide a seamless patient referral process. Collaborates with external and internal referral sources to establish standardized processes that ensure a smooth and accurate exchange of required demographic, clinical, and financial information required for the provision of requested Hospice and Home Care Services. 
  • Ensures Hospice and HH referrals are in full compliance with appropriate local, state, federal, national accreditation body rules, and regulations as well as agency policy and procedures are followed.  
  • Ensures workflow processes are timely and cost efficient. Functions as a resource to all others on eligibility and admission policies, procedures, and practices. 
  • Works collaboratively with Clinical Operations and other referral sources to establish and achieve performance, quality standards to improve the customer experience. This position is responsible for managing customer service complaints and resolving issues.
  • Establishes and maintains partnerships with BD, physicians, hospital leadership, external & internal providers, and care management to promote effective patient care transitions. 
  • Monitors and assures staff productivity according to established targets. 
  • Assists in management contracts for collaborative services to assure compliance with budget 
  • Ensures clinical documentation flows through the systems accurately ending in timely billing submission to payors. 
  • Develops collaborative and cooperative relationships with internal and external partners, entities and other customers, physicians, as well as community organizations. 
  • Responsible for understanding and adhering to the Advocate Aurora Health Care Code of Ethical Conduct and for ensuring that personal actions, and the actions of employees supervised, comply with policies, regulations, and laws applicable to Advocate Health
  • Supports leaders and oversees human resource activities and functions: recruitment, hiring, retention, pay decisions, performance appraisals, growth and development and corrective action. 

Licensure, Registration, And/Or Certification Required:

  • None Required.

Education Required:

  • Bachelor's Degree​ in Health administration, Business Administration or other health related fields.

Experience Required:

  • Typically requires 7 years of experience in Health care leadership experience in driving operational and/or strategic programs. Experience in a Home Health or hospice and post-acute setting is desired along with settings within a complex integrated health care system. 

Knowledge, Skills & Abilities Required:

  • Demonstrated ability to effectively manage a large team in a professional manner.
  • Demonstrated abilities in relationship building, communication and working collaboratively with leaders of varying functions and levels. 
  • Self-directed, results oriented, can think critically.
  • Thorough understanding of hospice and home health regulations, reimbursement, and care requirements. 
  • Ability to solve problems and make independent judgements and decisions based on experience and good judgement. 
  • Excellent team development abilities.
  • Excellent Computer Skills.

  • Thorough knowledge of organization product lines and services.
  • Ability to manage multiple sites and travel between sites. 
  • Demonstrated ability to organize, plan and delegate effectively and efficiently.

Physical Requirements And Working Conditions:

  • This position requires travel, therefore, will be exposed to weather and road conditions.
  • Operates all equipment necessary to perform the job.
  • Independent in working in a remote environment.
