The Hospice Central Intake and Triage Department is the main access point for all Providence Hospice customers (to include, but not limited to: current patients, current patient's family members, physicians, physician office staff, Inquiries, etc.). With new referrals, the Central Intake and Triage Nurse may arrange prescribed services with the physician, payor, patient/caregiver, and provider to ensure that services are provided in a timely and coordinated fashion. This position will also process triage calls from existing patients/caregivers, addressing clinical issues when appropriate and forwarding to individual team supervisors as necessary.
MAJOR CHALLENGES:
1. To efficiently utilize multiple computer on-line applications to complete referrals.
2. To establish and maintain positive working relationships with all Hospice Services customers.
3. To prioritize multiple tasks and work in a demanding, fast-paced active setting.
Patient Population Served: Adult, Geriatric
Consequently, the employee must be competent in the interpretation of a patient's self-report or behavior, and this information must be interpreted with an understanding of the cognitive, physical, emotional/psychosocial and chronological maturation process. The treatment of patients should be individualized and should reflect an understanding of the developmental needs and range of treatment needs for each patient.
Position Specific Responsibilities
- • Effectively represents the mission of Providence Health and Services in all aspects of the home services' central intake and coordination functions.• Takes referral information on-line and accesses other necessary information on-line (e.g. history and physicals, inpatient notes/summaries, and labs, and physician names and addresses) to complete the referral.• If a patient is deemed to require other services offered by Providence Home Services, the Central Intake and Triage Nurse will take the additional information and coordinate with the appropriate HS Department.• Identifies by telephone, patients that most like require hospice services and schedules an intake or assessment visit.• Works closely Hospice Insurance coordinator to investigate and document insurance eligibility and benefits and required authorizations for all referrals.• Communicates information to patients/caregivers, physicians, and providers and assists in making alternative arrangements as dictated by insurance coverage and benefits.• At time of referral, obtains complete insurance information on-line including name of insurance, name of insured person and relationship to patient, group and ID number of insurance, place of employment for insured member, and contact information for verification of insurance eligibility and benefits.• Works with hospital and extended care facility care managers and discharge planners, physician offices, and other referral sources to plan and organize home services for patient discharges in the local and regional service area.• Maintains accurate daily referral and on-call logs by referral and insurance source for tabulation of monthly referral/intake and productivity statistics.• Establishes and maintains positive working relationships with home care providers, payors/case managers, physicians and their office staff, hospital and extended care facility care managers and discharge planners, and Providence Home Services departments.• Focuses on being customer service oriented at all times.• Attends scheduled staff meetings twice per month and participates in department quality improvement initiatives and inservice education.• Performs other duties as assigned.
Education And/Or Experience
- Preferred - Bachelor's Degree Nursing or Social Work.
- Degree in related field may be considered.
- Preferred - Call Center experience.
Skills
- Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
- Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
- Computer Skills: Competency in keyboarding.
- Computer Skills: Knowledge and comfort with personal computers, a LAN, and various software packages required.
- Other Skills and Abilities: A comfort level in talking with both clinical and non-clinical (patients / family members) about hospice services.
- Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
- Other Skills and Abilities: An established record as a strong team player and the ability to work closely with others is required.
- Other Qualifications: An understanding of how the hospice care delivery system works and the interrelationships among patients, physicians, hospitals, home care providers preferred.
- Other Skills and Abilities: The ability to stay calm, focused and supportive when talking to persons (patients / family members / other hospice staff) dealing with difficult / stressful situations.
- Other Skills and Abilities: Requires strong organizational, analytical, and communication skills.
- Other Skills and Abilities: Also requires the ability to work independently, to coordinate multiple facets of patient care, to prioritize work, and to manage time to accomplish multiple tasks in an active office atmosphere with multiple interruptions.
- Other Skills and Abilities: Phone etiquette and strong customer service orientation essential.
Certifications, Licenses, and Registrations
- Required upon hire: Oregon Registered Nurse License (Vendor Managed) Or
- Required upon hire: Washington Registered Nurse License (Vendor Managed)
- Required upon hire: Registered Nurse License in state of hire, and wherever care is delivered.
Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential. Compensation range: $47.33 - $73.48
The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities
