About The Job

The SilverStone Social workers provide unique knowledge and psychosocial skills for seriously ill patients and their family both in Hospice & Palliative. Our clinicians often collaborate with social workers when caring for seriously ill patients.The Medicare Hospice Benefit and the National Consensus Project for Quality Palliative Care state that social workers are a core member of a hospice or palliative care (PC) interdisciplinary team (IDG). At SilverStone, the role of the social worker is one of patient and family advocacy, and it is of extreme importance.

Essential Duties and Responsibilities

  • Utilizing a biopsychosocial assessment of the patient and family as the basis for care planning
  • Assisting patients with advanced care directives
  • Patient/family psychoeducation regarding coping with illness, normalization of stress, and palliative care options such as hospice services
  • Resource identification and referrals; practical assistance; insurance coverage; system navigation; funeral planning.
  • Financial counseling related to the cost of care
  • Individual and family counseling specific to the disease process, coping, and planning
  • Assessing and addressing caregiving resources and needs
  • Crisis intervention including suicide assessment and prevention
  • Collaborating on discharge planning
  • Legacy work including dignity therapy and Acceptance and Commitment Therapy (ACT)
  • Bereavement care and grief counseling, including anticipatory responses to and processing of grief
  • Bachelor of Social Work (BSW) degree or Master of Science in Social Work (MSSW) degree.
  • Experience and certification in counseling.
  • Ideally, Clinical licensure at the Licensed Independent Clinical Social Worker (LICSW) / Licensed Clinical Social Worker (LCSW) level or similar
  • Ideally, certification in hospice and palliative
  • Must be willing and available take on-call shifts in rotation.
  • Excellent interpersonal skills and ability to communicate effectively.
  • Demonstrates proven decision-making skills.
  • Able to read, write and comprehend English.
  • Ideally knowledge of Home Care Home Base EMR Systems.

Environmental and Working Conditions

Works in patient home and skilled nursing facilities in various conditions; ability to work flexible schedule; ability to travel within Dallas County
