About the MyLife Project
The MyLife team empowers the voices of individuals and supports positive change, recognising that children, young people and families are the experts in their own lives.
As a multidisciplinary team, we provide seamless support across pathways, delivering individualised interventions funded primarily through Targeted Care Packages. These packages are designed to help young people transition from, or avoid entry into, residential services.
The Role – Case Manager
As a Case Manager with Anchor’s MyLife team, you’ll play a pivotal role in empowering young people to achieve their goals and live sustainable, independent lives. Working within a trauma-informed, client-centred framework, you will provide oversight, guidance and practical support in collaboration with Youth Workers, families and other care team members.
Key Responsibilities Include:
- Delivering services that respond to the individual needs of young people and families, with a focus on supporting transitions.
- Taking responsibility for all aspects of contracted case management in line with statutory requirements.
- Working in genuine partnership with clients, families, Youth Workers and the wider care team.
- Actively using program tools and assessments to track outcomes and inform practice.
About You
You are an experienced practitioner with specialist knowledge of trauma-informed practice and the statutory system. You thrive in collaborative environments and are committed to seeing young people through a lens of potential, not deficit.
You Will Bring:
- Demonstrated experience in case management with children, young people and families.
- A sound understanding of trauma, positive psychology and developmental frameworks.
- The ability to work constructively with young people from diverse and culturally and linguistically diverse backgrounds.
- Strong organisational skills and the capacity to balance multiple priorities.
