Job Description:
The community Infection Prevention and Control (IPAC) Hub is an integral part of Trillium Health Partners. As a member of this department, the IPAC practitioner facilitates infection control activities in Long term Care, Retirement homes, and other congregate living settings (CLSs) including residential settings funded by the Ministry of Health (MOH), Ministry of Children, Community and Social Services (MCCSS) shelters, and supportive housing in the THP catchment area, The hub is considered an agent of change in the THP community, through ongoing advocacy for IPAC best practices in CLSs with the primary goal of reducing the risk of healthcare-associated infections.
The successful IPAC practitioner will directly report to the IPAC Hub Manager and will be expected to support multiple facilities as assigned.
Responsibilities:
- Development and implementation of outbreak management plans, in conjunction with public health partners and CLSs
- Provide IPAC expertise and support to CLSs.
- Deliver IPAC education and training.
- Support the development of IPAC programs, policies and procedures within sites and organizations.
- Support assessments, audits and provide recommendations to strengthen IPAC programs and practice.
- Mentor those with responsibilities for IPAC within CLSs.
- Support CLSs to implement IPAC recommendations.
- Other requirement: Car and full drivers license for onsite visits to CLSs
Qualifications:
- Minimum of 3 years’ divers experience in Infection Prevention and Control in community and/or hospital settings
- Current registration with the Certification Board of Infection Control and Epidemiology (CIC)
- Degree in health-related discipline (e.g. nursing, laboratory, epidemiology, and other allied health) or equivalent combination of education and experience
- Demonstrates strong IPAC knowledge and experience in related areas of infectious diseases, microbiology, epidemiology, statistics, asepsis; disinfection/sterilization; outbreak management, HVAC & construction
- Demonstrated skills in coaching, mentoring and excels in adult education
- Strong written/verbal communication skills
- Critical thinking in complex and stressful situations, conflict resolution skills, high emotional intelligence
- Proficiency in the use of computer databases, spreadsheets and literature search, familiarity is an asset
- Ability to work independently, self-directed and able to effectively lead and provide support to clinical & non-clinical teams to manage complex and challenging situations
- Demonstrated ability to work collaboratively in multidisciplinary team environment
- Self-directed, well-organized and detail-oriented
