About The Role:
We have an exciting vacancy for a Case Manager to join our Homelessness East Metro team in this full time, 12 month, fixed term vacancy. Remuneration in line with SCHADS Level 5.
This dual-role position splits time between:
- Homeless Outreach Program Case Manager (3 days per week): providing case management, system navigation, housing information, advocacy, and connections to community supports for people experiencing homelessness or at risk of homelessness, with a focus on rough sleepers and those disconnected from mainstream services.
- Care Finder (2 days per week): delivering specialist assistance to help older community members access aged care and other services, including information provision, advocacy, and community linkages.
Key Responsibilities:
- Deliver quality, holistic case management and information services under the National Homelessness Stream Model of Care and funding guidelines.
- Conduct initial assessments, develop and review case plans, provide interim support, and support case closure.
- Actively engage with clients to address immediate housing and support needs; navigate services; foster self-determination and participation.
- Develop and maintain professional relationships with clients, internal programs, and external partners; advocate for clients’ needs.
- Ensure inclusion and safety for LGBTIQA+ clients; maintain culturally safe practices.
- Maintain accurate client records and documentation; complete reporting and data collection as required.
- Participate in TIC (trauma-informed care) training, supervision, and performance development; contribute to a supportive team environment.
About You:
Essential:
- Appropriate degree in Social Work, Psychology, Occupational Therapy, or equivalent (Diploma with strong relevant experience considered).
- Demonstrated case management experience (approximately 1 year or more).
- Experience in community services and working with vulnerable populations, including adults experiencing homelessness, AOD, mental health, disability, and family violence.
- Ability to work effectively in outreach settings and engage clients with complex needs.
- Knowledge of relevant theoretical frameworks and trauma-informed approaches.
- Proven ability to meet program targets, manage challenging behaviours, and respond to crises.
- A National Police Record Check.
- A current and valid Working with Children Check (Employee).
- A current and valid Australian Driver’s License.
