STATUS: Non-exempt, full-time, 37.5 hrs/wk
HOURLY RATE: $21.50 to 23.00 per hour
Summary:

Lifespan Care Managers link older adults and caregivers to services and opportunities that are available within the community and offer a holistic assessment of needs by identifying health, financial, emotional, functional and environmental concerns. Care Managers offer care options, solutions and guidance navigating challenging systems. Clients are met with in-person and virtually. There are no after-hours or on-call responsibilities.
Duties And Responsibilities
- Accept referrals as assigned.
- Completes comprehensive in-home assessments to determine needs.
- Creates and implements care plans that meet client needs and preferences.
- Reassesses client needs and revises care plans as needed.
- Assist with connecting to community-based programs and resources.
- Complete database documentation in an accurate, timely fashion.
- Maintain a professional knowledge of services and meet core skill competencies.
- Participate in agency/team meetings, cultural competency, and corporate compliance training.
- Accepts and seeks guidance and clinical supervision.
- Other relevant duties assigned by the supervisor.
Requirements
Qualifications
Education: Bachelor or Master of Social Work or equivalent degree
Experience: Two years in care management or another human services role, with at least one year with
Older Adults/Caregivers.
Additional Requirements
Ability to work in-office.
Ability to meet with clients in their home.
Proficiency with Microsoft Office products and Zoom.
Strong Organizational Skills.
Excellent verbal and written communication skills.
Ability to work in multi-cultural situations.
Ability to handle stressful situations in a professional and respectful manner.
Valid NYS driver’s license and reliable vehicle.
