NP or PA - Integrated Family Care Clinic - Fort Atkinson

Fort HealthCare's Integrated Family Care Clinic is seeking a full-time, Advanced Practice Nurse Practitioner or Physician Assistant. The nurse practitioner or physician assistant provides a broad range of primary health care services for various ages beginning at infants up to elderly adults. The focus of this position is wellness, prevention and health promotion. The nurse practitioner or physician assistant in collaboration with the primary physician, performs physical examinations and developmental assessments; treats common acute illnesses and minor medical conditions of all ages and helps families meet their health needs.  

Shift:
Full Time
Day Shift
32 patient contact hours/week. 

Requirements are a Bachelor’s Degree, graduate of a Physician Assistant program and WI license or Registered Nurse with current Wisconsin license upon hire; APNP certification, prescriptive authority; BLS certification. Experience in a physician's clinic preferred.
