Collective Health is looking for a full time, experienced, and forward - looking Social Worker to guide and support members facing complex conditions driven by data, shaped by evidence-based best practices, and supported by world-class customer service. The SW Care Managers are part of an interdisciplinary care team located in Collective Health’s Plano, TX office. The SW Care Manager will ensure that our members’ care is optimally coordinated.
What You'Ll Do:
- Leading outreach efforts to high-risk individuals with complex medical conditions through telephonic or electronic outreach
- Completing telephonic social work assessments and care plans for patients with behavioral, social, financial and environment problems
- Educating and empowering members using decision support through multiple modalities including telephone and email to help members achieve their health goals
- Identifying and discussing current barriers related to member’s current condition, and provide interventions to address these barriers
- Providing emotional support, assisting with benefits and behavioral health navigation
- Coordinating a range of services, including community resources and health benefit resources
- Utilizing motivational interviewing to assist members to become self sufficient in their care planning
- Participating in clinical interdisciplinary rounds
- Working collaboratively on co-managed cases with nurses, pharmacists, dietitians, and care coordinators
- Working cross functionally with other teams to assist with benefits and claims questions
- Will be required to take call as scheduled
- Required Travel: 1-2 days per quarter with client-facing and member-facing presentation and interaction; travel will be within Texas
- Other duties as assigned
- Spend 85% of your time in the direct case management role and 15% on project work and other opportunities as they are available
To Be Successful In This Role, You'Ll Need:
- An LMSW or LCSW from an accredited school of Social Work and are licensed in the state of Texas
- To not require hours of supervision for licensure
- Experience in case management; experience at a payer is a plus
- Bilingual Required (English/Spanish)
- To be passionate, empathetic, and excited to help people
- To be independent, tenacious, and capable of identifying and implementing best practices
- To be capable of thinking creatively to implement care navigation solutions that challenge status quo
- To be driven, able to carry a caseload and prioritize to complete daily tasks and outreach
- To be tech savvy; able to talk, type, and navigate an electronic chart with ease
- To be proficient in Google suite, such as Google Sheets, Docs, etc.
- To be excited to work in a telephonic social work role
- To be okay with a role that does not involve providing individual therapy
- To be a problem solver using best practices to achieve goals
- To be comfortable with ambiguity and open to collaborating with people across different teams
- Additional Case Management Certification is a plus
- Discharge planning, complex care coordination, behavioral health, and/or home health, oncology, behavioral health, autism, pediatrics, and/or maternity experience a plus
- A strong moral compass and commitment to Collective Health’s core values of integrity, transparency and trust
- To work in office 3 days a week
- Collective Health will not be able to provide supervision for clinical hours
- Collective Health will not provide supervision or experience with Clinical Psychosocial Diagnosis, Assessment, Treatment, Individual or Group Psychotherapy
Pay Transparency Statement
This is a hybrid position based out of our Plano office, with the expectation of being in office at least 3 weekdays per week. #LI-hybrid
The actual pay rate offered within the range will depend on factors including geographic location, qualifications, experience, and internal equity.
