Your Opportunity

Drug and Alcohol Youth Service (DAYS) Aftercare provides case coordination and support to young people as they transition from DAYS residential programs including the Youth Withdrawal and Respite Service, the Residential Rehabilitation Service, or THASP. Aftercare works to sustain progress made at DAYS and minimise relapse of Alcohol and Other Drugs, while engaging young people to build independence through stable accommodation, vocational pathways, and social connection. The role also provides outreach case coordination, counselling, and group facilitation to support recovery and community integration.

The Aftercare AOD Case Manager is an on-site position and plays a vital role in ensuring continued progress for individuals who have completed the residential program, helping them sustain positive outcomes, minimise relapse, and address their physical, social, and emotional needs as they transition back into the community.

Your Key Responsibilities

  • Provide quality, client-centered case management practices that are culturally safe and aligned with Mission Australia’s National Case Management Approach.
  • Build strong, trusting relationships with young people to support engagement in treatment, recovery, and community participation.
  • Collaborate with clients to develop holistic, strength-based case plans using SMART goals, ensuring appropriate referrals to programs and services.
  • Assist young people in transitioning to independence or other support services, including building capacity for self-management and sustainable recovery.
  • Advocate for young people and deliver AOD counselling and harm minimisation education to clients, families, and external agencies.
  • Maintain accurate case notes, assessments, risk management plans, and reports while ensuring timely data entry and program compliance.
  • Establish and maintain strong external networks, attend sector meetings, and collaborate with internal and external stakeholders to enhance service outcomes.

Requirements For Success

  • A Degree or Diploma in Social Welfare, Social Science, Youth Work, or a related field, or substantial experience in the Alcohol and Other Drug (AOD), child, youth, or family sectors.
  • Strong knowledge of child and adolescent development, youth safety practices, and the complexities of AOD-related issues.
  • Demonstrated experience managing a caseload of clients with complex needs, applying case management approaches and trauma-informed assessment processes.
  • Proven ability to engage, motivate, and maintain professional relationships with young people and stakeholders, with excellent verbal and written communication skills.
  • Highly developed organisational and time management skills, with competency in report writing, maintaining client records, and using computer-based systems including Microsoft Office.
  • Flexibility, resilience, and innovation in providing outreach support and sustaining engagement with at-risk youth, including addressing vicarious trauma.
  • Ability to work both autonomously and collaboratively within a multidisciplinary team, maintaining professional boundaries and contributing to positive service outcomes.
  • First Aid Certificate is desired but not essential.
  • Full driver licence with no restrictions.
