High Country Community Health (HCCH), a mission-driven Federally Qualified Health Center (FQHC) serving rural communities in western North Carolina, is seeking a Clinical Pharmacist to serve as Director of Complex Care Management. This leadership role is ideal for a pharmacist passionate about improving outcomes for patients with multiple chronic conditions and social

complexity in underserved, rural areas.

The Director will oversee care coordination for high-risk populations and advance HCCH’s participation in value-based care, advanced primary care models, and whole-person care delivery, collaborating closely with medical, behavioral health, dental, and pharmacy teams.

Specific Responsibilities Include:

Program + Team Leadership:

  • Lead and continuously improve HCCH’s Complex Care Management (CCM) program, focusing on patients with high medical, behavioral, and social needs.
  • Supervise a multidisciplinary team including care coordinators, community health workers, and nursing support.
  • Implement evidence-based models for rural Advanced Primary Care, including Patient- Centered Medical Home (PCMH) principles, transitions of care, and chronic care management.

Clinical Pharmacy Services:

  • Deliver Comprehensive Medication Management (CMM) services within primary care workflows.
  • Serve as a pharmacotherapy consultant to medical providers for chronic disease management, high-risk medication review, and deprescribing strategies.
  • Provide medication counseling and support adherence, especially for patients with limited health literacy or access barriers.
  • Work with HCCH in house 340B pharmacy to maximize cost effectiveness and medication access for patients.

Value-Based Care + Population Health:

-Align care management and pharmacy strategies with HCCH’s value-based contracts

(e.g., ACOs, Medicaid Managed Care, HRSA metrics).

-Use population health analytics to stratify risk, identify care gaps, and support quality

improvement efforts (e.g., HEDIS, UDS, 340B optimization).

-Participate in reporting and documentation for payer incentive programs and state

Medicaid Initiatives.

Requirements

Requirements/Qualifications/Skills/Experience

  • Minimum of 3 years of clinical pharmacy experience in ambulatory care or rural health.
  • Experience in care coordination, complex case management, or integrated team-based care.
  • Prior involvement in value-based care, quality programs, or advanced primary care strongly preferred.

Knowledge, & Abilities:

  • Proficiency in managing chronic disease states such as diabetes, hypertension, CHF, and behavioral health conditions.
  • Working knowledge of rural health disparities, health equity, and social services

Navigation.

  • Familiarity with EHR systems (e.g., eClinicalWorks, EPIC) and data reporting tools.
  • Strong leadership, communication, and interdisciplinary collaboration skills.

Preferred Certifications:

-BCACP (Board Certified Ambulatory Care Pharmacist), BCGP (Geriatrics), or CDCES

(Diabetes Care and Education Specialist).

-Certification in Care Management or Population Health is a plus.

Travel Requirements

Travel for continuing education/training as needed (at least yearly). Primary location in Boone,

NC, with occasional travel to other HCCH sites in Burke, Avery, and Surry counties.

Salary Range- Negotiable

Work Hours

Full-time salaried: 40 hours/week (Monday through Friday)
