Our network of small local teams enables Chorus people to be more connected to each other and feel a sense of belonging.
About The Role
- We are seeking 2 Registered Nurses – one based north of the river and one south of the river.
- Work closely with our local teams to coordinate and deliver high-quality in-home care.
- Conduct clinical assessments and provide clinical care services.
- Develop care plans, in consultation with customers and local teams, that reflect individual goals and needs.
- Communicate effectively with local teams and others involved in the customer’s care.
- Monitor quality, safety, and compliance with clinical standards and regulatory requirements.
- Use technology and digital systems to streamline processes and maintain accurate, up-to-date records.
About You
You are a qualified Registered Nurse with a passion for community care and a commitment to supporting people to live independently.
- Demonstrated experience in home care.
- Strong clinical assessment and care planning skills.
- Excellent coordination and communication abilities.
- Confidence in using digital platforms and technology to support efficient care delivery.
- A collaborative, customer-centred approach with a focus on continuous improvement.

- Be the clinical heart of our Local teams, bringing your nursing expertise to bear in creative, person-centred ways to support customer centred clinical care
- Build genuine relationships with customers and local teams, walking alongside them on their journey rather than just delivering services
- Think outside the clinical box – work with locals to develop care plans that connect people to their community, not just address their medical needs
- Embrace technology that makes life easier (not harder!) for both you and your customers
- Work autonomously within a supportive framework - no micromanagement, just clear agreements and the trust to deliver

Who You Are
The Non-Negotiables:
- Registered Nurse with AHPRA registration
- Experienced in community or home-based care
- Valid driver's license, your own vehicle available to use for work related travel and love of building relationships in teams and the community Genuine belief that everyone deserves to belong
