Opportunity awaits! Opportunity awaits! We are hiring multiple Case Managers (Human Services Case Manager) to join our Aging and People with Disabilities (APD) team. In this position, you will assess Oregonians applying for, or currently receiving, long-term care services to develop, coordinate, and arrange a variety of services to allow older adults and people with disabilities to get the continuity of care they need. Summary Of Duties

As a Case Manager, you will:

  • Determine long-term care program eligibility by interviewing Oregonians to determine initial and ongoing eligibility for healthcare, social services, and protective service needs.
  • Develop detailed, individualized, and person-centered case planning for Oregonians.
  • Conduct interviews in person/on site using the Case Assessment and Planning System (CA/PS) assessment tool on a state-issued laptop.
  • Inform Oregonians of rights, responsibilities, service options, and methods of recourse.
  • Make appropriate referrals to other ODHS programs and community resources.
  • Process applications, inquiries, and data according to policy timeframes, ensure appropriate documentation and forms are completed and sent, and communicate eligibility decisions to ensure delivery of benefits and services.
  • Provide intake case management by coordinating the provision of long-term care services to Oregonians according to rules, regulations, and the individual case plans.
  • Contact Oregonians or collateral contacts monthly to set up and monitor service plan and safety needs. This could include in person, the phone, or electronic contact.

Minimum Qualifications

A valid driver’s license and acceptable driving record are required for this position.

A bachelor's degree in a Behavioral Science, Social Science, or a closely related field

Or

A bachelor's degree in any field AND one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing)

Or

An associate degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing)

Or

Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).

Essential Attributes

We are looking for candidates with:

  • Experience determining eligibility and appropriate levels of service.
  • Experience interpreting and implementing policies and procedures.
  • Experience interviewing to obtain information to evaluate needs and next steps in the intake process.
  • Proficient in using computer software such: Microsoft Office, Word, Excel, Outlook, web browsing and communication applications.
  • Spanish/English bilingual skills encouraged.
  • Collecting, analyzing, and evaluating data while navigating databases for information.

Working Conditions

  • Your work schedule will be Monday through Friday from 8am-5pm.
  • After 6 months of trial service, your work schedule may be flexible with option to work between 6am-6pm.
  • Work schedule specifics along with possible teleworking options will be determined by the hiring supervisor.
  • Fast paced environment with constantly evolving policies and procedures.
  • May drive a variety of different types of vehicles to remote places in a variety of terrain and possibly inclement weather.
  • Repetitive motions, prolonged sitting, and daily computer and technology use. 
  • Will come in contact with animals, some which of may be dangerous.  
  • May be exposed to communicable diseases, such as the flu virus, Covid-19, and MRSA.
  • You will interact with a wide range of people including some who have experienced trauma and may exhibit challenges in controlling their emotions.

General Information

  • This is a permanent, full-time position and is represented by the Service Employees International Union (SEIU).  
  • This recruitment may be used to fill future vacancies in the same classification.
  • Salary Information: Public Employee Retirement System (PERS) members in the executive branch (except AEE and AFSCME Parole & Probation) will get a 6.95% base salary increase and will pay a 6% employee contribution to PERS. Employees who are not yet PERS members (AA) will also get the 6.95% pay increase and start the 6% contribution when they join PERS (AP).

The step you start at will depend on the Equal Pay Analysis for the Human Services Case Manager:

AA: $4,409 - $6,420  

AP: $4,715 - $6,866
