Position Purpose.

The Behavioral Health Case Management Coordinator plays a vital administrative role in supporting the Behavioral Health Case Management team. This position is responsible for receiving, reviewing, and routing referrals to licensed behavioral health case managers for member engagement and follow-up. The coordinator serves as a key point of contact for members, providers, and internal departments, ensuring timely and accurate communication and coordination of services. The coordinator also assists with data collection and analysis of referral and service volume for internal and external reporting. 

How You Contribute to the Company’s Mission In This Role

  • Receive and review behavioral health case management referrals from multiple sources including self-referrals, providers, customer service, Utilization Management, and medical case management programs via the Epic system and other sources.
  • Triage and route referrals to the appropriate licensed case manager based on member needs and program criteria.
  • Contact members to clarify referral details when necessary.
  • Respond to inbound calls from members, providers, and internal departments regarding behavioral health services.
  • Send initial communications such as welcome letters, general information packets, and follow-up correspondence.
  • Assist case managers with scheduling of appointments.
  • Conduct member satisfaction surveys post-engagement.
  • Collect and compile survey data for reporting and quality improvement initiatives.
  • Assist licensed case managers to locate community resources and support services for members.
  • Maintain accurate documentation of referral activities and member interactions in the case management system.
  • Ensure compliance with regulatory and organizational standards for privacy and documentation.
  • Works closely with management and staff to participate in operational activities, research and implementation of program enhancements, compliance monitoring, and accreditation.
  • Assists with meaningful member outreach activities, assisting with targeted outreach projects to members and providers that will enhance member engagement

Required Qualifications

  • High school diploma or equivalent required.
  • Minimum of 3 years of experience in a healthcare or insurance setting, preferably in behavioral health or case management coordination.
  • Requires the demonstrated ability to identify, make recommendations and take action on opportunities and potential operational efficiencies within the unit and offer solutions and recommendations to management.
  • Ability to adhere to defined metrics for monitoring work and preparation of reports to reflect work efforts are required to complete tasks/projects
  • Familiarity with Epic or similar electronic health record systems
  • Strong organizational and multitasking abilities.
  • Excellent verbal and written communication skills.
  • Proficient in Microsoft Office Suite and case management software.

  • Ability to work independently and collaboratively in a fast-paced environment.
  • Empathy and professionalism in member interactions.
