As a Case Manager, you’ll connect with prospective residents, clients, families and advocates, ensuring they feel valued and supported. Through expert assessments, you’ll maximise services, partner with internal and external stakeholders, and ensure clients’ clinical needs and preferences are met. You’ll also contribute to the growth of Seniors Living by showcasing Anglicare’s unique approach and seizing new opportunities.

Your Opportunity

Part-time (20 hours per week), Kirrawee Location

With care and professionalism, you will:

  • Provide exceptional customer service, assess client needs, and create personalised care plans
  • Guide clients from initial contact to commencement of care, building trust and credibility
  • Regularly assess and advocate for clients to maximise services and support
  • Maintain accurate records in CRM and IT systems
  • Build strong relationships with stakeholders to support client outcomes

What You’Ll Bring

You’re a proactive and compassionate professional who thrives on delivering high-quality case management and creating meaningful client relationships.

You Will Need:

  • Diploma in Service Coordination/Community Services or equivalent
  • Experience in the aged care sector, particularly with home assessments
  • Exceptional customer service and sales skills
  • Strong communication, organisation, and IT skills
  • Current Driver’S Licence
