Logan Health's Cancer Care Program is looking for an Oncology Social Worker to join their team!

What You'Ll Be Doing:

As an Oncology Social Worker at Logan Health, you’ll play a vital role in supporting patients, families, and caregivers across our Hematology/Oncology Clinic, Radiation Oncology, and Oncology Infusion areas. In this role, you will:

  • Provide resources, support, and follow-up for patients navigating complex care and positive results
  • Offer emotional, grief support, and advocacy to help patients and caregivers overcome barriers in the healthcare system
  • Participate in program activities, events, and outreach that strengthen the Cancer Care Program
  • Collaborate with an interdisciplinary team to improve the patient experience and outcomes
  • Split your time between direct patient encounters and programmatic work, including community engagement and outreach
  • Who We’re Looking For:
  • We’re seeking a seasoned social worker who thrives in both clinical and program settings. The ideal candidate will bring:
  • Solid clinical social work experience
  • Background in healthcare, ideally in oncology or a related setting
  • Experience or exposure to grief counseling and support services
  • Strong awareness of local and regional community resources
  • The ability to work independently while collaborating effectively with a diverse care team
  • A passion for helping patients and families navigate challenging healthcare journeys with dignity and compassion

Qualifications:

  • Master’s Degree in Social Work.
  • Current Montana LCSW or LMSW licensure highly preferred.
  • OSW-C certification preferred or willingness to obtain
  • Experience in a medical setting preferred. 
  • Current CPR per assigned area(s) may be required.
  • Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
  • Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
  • Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.

Other Job Specific Duties:

  • As applicable to assigned area(s); Addresses identified support and social needs of patients including, but not limited to: trauma, palliative care, comfort care/end of life patients, patients with substance use disorder, patients with complex social situations, patients with active psychiatric and/or behavioral concerns, situations with Guardianship and other legal issues impacting health and recovery.
  • Exhibits attentiveness of risk for readmission and works with the interdisciplinary team across the healthcare continuum to address social determinants of health that may impact the discharge planning process.  Keeps the patient and identified support person(s) engaged in all aspects of planning.
  • Explores and communicates solutions and recommendations to social barriers impacting healthcare with appropriate stakeholders Seeks input from the multidisciplinary team. Completes thorough and accurate documentation in the patient’s medical record.
  • Exhibits knowledge and skills necessary to provide for the physical, psychological, and cultural needs of the patient.  Advocates for clients' rights to self-determination, confidentiality, and access to supportive services and resources, and appropriate inclusion in informed decision-making that affects health and well-being.
  • Annually completes one (1) organization-wide teaching event or one (1) department-wide training session specific to Social Work.
  • In collaboration with leadership, identifies and assumes responsibility of one (1) existing or needed Case Management policy that is specific to Social Work, to review and update annually.
  • Consistently reflects ownership and professionalism towards Care Coordination. Identifies areas in need of improvement and serves an active role in developing solutions. Exhibits the Organization’s Core Values to both internal and external customers.
  • Responds to, and provides direction in, a changing workload in a positive and professional manner.

Shift:

Day Shift - Variable Hours (United States of America)

Logan Health operates 24 hours per day, seven days per week.  Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
