External Care Manager

External Care Manager - ECM

This position is responsible for overseeing caregivers in the field with their client and ensuring care is being provided in a professional, caring and safe manner.  This position will work with their client, family and staff to develop a careplan and task list for staff working with each client.  They will maintain a client load and oversee client satisfaction with the caregivers working in the home, our services and overall agency experience.

Additional Duties

• Observes proper conduct while in resident’s homes.
• Adheres to Agency’s infection control and safety policies.
• Attends Agency’s in-service programs and staff meetings as needed.
• Assists in orientation of new personnel as requested.
• Respects client’s beliefs, cultures and limitations without prejudice.
• Practices effective cost-containment and accountability with supplies and equipment.
• Maintain an on-going audit of clients assigned to ECM

Experience Required

• Minimum of 2 years’ experience in a health care setting, preferably home care.
• Self-directed, independent worker.
• Ability to communicate effectively orally and in written form with residents, families, staff, vendors and the general public.
• Must have compassion and desire to work with the elderly.
• Must meet all health requirements including TB and background check.
• Maintains necessary licensure and certifications as required.
• Prefer Medical Assistant or Certified Nursing Assistant; or equivalent experience
