Job Summary
The Lung Nodule Navigator RN optimizes the patient experience while improving outcomes and enhancing patient satisfaction by providing seamless patient-centered navigation from incident findings to cancer care continuum from initial diagnosis until transition into survivorship or end-of-life care. Navigators provide high quality, timely care through collaboration with physicians and other members of the interdisciplinary care team to reduce real and/or perceived barriers to receiving their needed care. The navigator facilitates care coordination with incidental findings for referrals to specialists, appointment scheduling, and medical record review and ensures that patients have access to social services, nutritional support, financial counseling, clinical trials, genetic counseling, education, and resources at Hamilton and in the surrounding community. Provides education regarding lung nodules, potential diagnostic testing, and treatment pathways.
Collaborates with the cancer registry to ensure accurate reporting and data tracking. Participates in quality improvement initiatives and contributes to program growth and community outreach. The navigator serves as a patient’s point of contact throughout the cancer care continuum, consistently promoting patient advocacy, addressing patient/family concerns, and assuring high-quality care.
Qualifications
Job Qualifictions
Education: BSN required. ADN RN required obtaining BSN within 12 to 18 months.
Licensure: License as a registered professional nurse in the State of Georgia, eNLC license required. BLS CPR required.
Experience: Minimum of 3 years of nursing experience in oncology.
Certification: Qualifying healthcare certifications must include patient navigation within their exam to meet the measure of compliance for this standard, and possess one of the following certifications within 3 years of hire.
- Oncology Certified Nurse (OCN®)
- Oncology Nurse Navigator-Certified Generalist (ONN-CG™)
Training Program: Qualifying training programs must include competency-based training and education in patient navigation for patients with breast disease or breast cancer to meet the measure of compliance for this standard. Examples of such qualifying training programs include, but are not limited to:
- Oncology Nursing Society: Equipping the Novice Oncology Nurse Navigator: An ONS Collaboration with AONN+
- American Cancer Society Leadership in Oncology Navigation (ACS LIONTM)
Skills:
- Knowledge of self-care strategies and resources
- Strong skills in critical thinking and communication
- Ability to develop collaborative relationships both internally and externally
- Ability to increase patient/caregiver knowledge and self-determination
- Ability to work independently as a team member
- Strong organizational skills, ability to prioritize and adapt quickly to changing situations
- Ability to work within a value-based care framework, identifying the most appropriate resources to help the patient receive seamless care
- Basic computer skills- Microsoft Word, Excel, and database required to assess the performance of the program
