About The Program

Thorne Harbour Health has a vacancy for a Home Care Worker to join our Community Support team, working in the central or south-eastern region of Melbourne.

The Home Care program is a key aspect of our Community Support program, based at the Positive Living Centre (PLC) in South Yarra. The PLC provides a range of health and wellbeing related programs for people living with HIV.

About The Role

As a Home Care Worker you’ll be delivering home-based support services for people living with HIV who need assistance to continue living independently in the community. The focus is on engaging clients and enabling them to develop their own skills and capacity.

You will also be working with people in the LGBTIQ+ community who have an aged care package or are participants in the National Disability Insurance Scheme (NDIS) and need help at home or assistance with community participation.

Role Responsibilities Include:

  • Attending client homes at pre-arranged times to complete shifts, usually two to three hours.
  • Completing activities specified in each client’s support plan, including providing assistance with shopping, cooking, cleaning, and community engagement.

The role requires attendance at regular supervision sessions with support staff and training sessions as required.

About You

We’re looking for a warm and empathic, community-minded support worker, who is able to work both independently and as part of a team.

You Will Also Have:

  • Certificate III in home and community care, individual support, aged care, or similar qualification.
  • Your own car and current driver licence.
  • Strong interpersonal skills, including verbal and listening skills.
  • Ability to develop rapport with clients from a wide range of backgrounds and with varying physical and mental health issues.
  • Understanding of professional boundaries and ability to maintain the boundaries of the role.
  • An interest and willingness to work with people from LGBTIQ+ communities and people living with HIV.

Experience in attendant care work and knowledge of the HIV sector and related community services are both desirable.
