Clinical Pharmacist

As a Clinical Pharmacist, you will be responsible for reviewing medication plans, counseling patients, collaborating with healthcare providers, and contributing to the overall improvement of pharmacotherapy outcomes. Your work will directly influence patient safety and quality of care, making you an essential member of our healthcare team.

Key Responsibilities

• Evaluate medication orders for safety, effectiveness, and appropriateness
• Provide comprehensive patient education regarding medications, including their risks, benefits, and adherence
• Collaborate with physicians and healthcare professionals to optimize medication regimens
• Perform medication therapy management and therapeutic drug monitoring
• Participate in patient care rounds and contribute to interdisciplinary discussions
• Document clinical interventions accurately and maintain up-to-date patient records
• Engage in continuous professional development and stay current with pharmacy practices and guidelines

Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited institution
• Current pharmacy license in the state of practice
• Previous clinical pharmacy experience is preferred
• Strong understanding of pharmacotherapy and clinical guidelines
• Excellent communication and interpersonal skills
• Analytical thinking and problem-solving capabilities
• Ability to work collaboratively in a fast-paced environment
