At Hi-Desert Medical Center (HDMC) we've been caring for our community for over 40 years. Our Continuing Care Center (CCC), located on the campus of HDMC, provides specialized care to residents close to their own communities, with spectacular panoramic views of desert landscapes for both residents and their guests 

The Continuing Care Center has 120 beds and offers three distinct levels of service in semi-private rooms: 

Subacute care: This is the highest level of care available in our long-term care setting. The CCC provides 25 beds dedicated to residents with ventilator and tracheostomy requirements. Clinical staff is specially trained to meet the unique needs of these patients. The CCC is one of the few facilities in Southern California providing this level of care. 

Skilled nursing care: We provide long-term skilled nursing care to residents who require assistance with personal and medical needs. Care is provided to our patients who require rehabilitative therapies, wound care or recovery from accidents or invasive procedures. 

Long-Term Care: We provide long-term care to residents who require assistance with personal and medical needs. 

Summary

Organizes and conducts occupational therapy programs in the hospital to facilitate rehabilitation in the acute and skilled nursing settings. Directs patient participation in selected tasks to restore, reinforce, and enhance performance; facilitates the learning of those skills and functions essential for adaptation and productivity.

  • Shift: 7:30am - 4:00pm
  • Hours: 8hrs
  • Schedule: Monday thru Friday

Primary Duties

  1. Plans a variety of sensory motor, educational, recreational and social activities to provide occupational therapy to patients.
  2. Consults with other members of the rehabilitation team to select the most appropriate activity program for individual patients, based on patient needs and capabilities.
  3. Selects appropriate activities based on individual’s physical capacity intelligence level and interest. 
  4. Teaches patient the skills and techniques required for participation in occupational therapy; evaluates patient progress, attitudes, and behaviors on an ongoing basis.
  5.  Evaluates patient’s progress, attitudes and behaviors on an ongoing basis.
  6. Prepares discharge report summarizing patient’s reactions to therapy.
  7. Conducts internal training programs for agency staff on occupational therapy techniques and objectives. 
  8. Designs and fits adaptive devices such as splints and braces for patient use, based on patient disability and rehabilitation needs.
  9. Consults with physicians on patient’s progress; maintains appropriate records.
  10. Maintains established departmental policies, procedures, objectives, and quality assurance.
  11. Performs above primary duties for ages adolescent thru older adult.

Physical Demands:

  • Occasional walking and standing 
  • Lift supplies/equipment/ and or transfers patients 
  • Moderate physical effort (lift/carry up to 25 pounds)
  • Occasional reaching, stooping, bending, kneeling and crouching\

Minimum Education/Certification/Experience:

  • Occupational Therapy License
  • Current CPR license
  • Current Driver’s License
