Ritter Center is currently recruiting a Clinical Coordinator for our Clinic Team. This full-time, non-exempt position reports to the Clinic Manager based in San Rafael, CA; the role operates out of our main Campus.

The Clinical Coordinator functions as 50% Medical & Administrative and 50% Coordinator for all Ritter Health Center patients, coordinating care among providers and agencies, and facilitating the development of a plan of care which extends beyond the primary care visit. This role acts as a culturally competent, comprehensive care coordinator for patients assigned to their care, assessing the patients’ needs and facilitating communication amongst other clinicians and agencies providing care. This position is responsible for patient registration, checking insurance eligibility, scanning ID’s, scanning faxes into the electronic health record, and collecting Insurance co-pays. This person will also support senior managers with scheduling and any other administrative needs.
Essential Functions and Responsibilities
Clinical Front Office (40%)
- Admit patients and maintain patient flow. Help to answer incoming clinic calls and related administrative and paperwork duties to ensure efficient check in and check out processes including scheduling.
- Entry of patient clinical and non-clinical information into electronic health records system (e-Clinicalworks), eCW, EHR system, file records (insurance cards, registration form, ID, referral reports, etc) in appropriate sections of patient charts and keeps patient charts in proper order
- Proactively work with Chief Medical Officer and Clinic Manager to resolve any technical or flow issues regarding referrals and front office.
- Demonstrate appropriate judgment skills to be able to make independent clinical decisions in routine patient care matters
- Provide a warm greeting and top-tier customer service to every patient
- Check in and collect appropriate documentation from patients
- Per provider instructions, operate and maintain medical equipment
- Schedule patient appointments and procedures according to established policies and protocols
- Understand HMO, PPO, Medicare and Medi-cal insurance plans in order to collect appropriate out of pocket expenses for patients
- Utilizing eCW EHR system, file records (insurance cards, registration form, ID, referral reports, etc) in appropriate sections of patient charts and keeps patient charts in proper order
- Understand HIPAA in order to maintain patient confidentiality
- Meet with supervisor; attend staff meetings, and trainings as scheduled
Referral Coordination (50% time)
- Manage and coordinate all referrals generated in Ritter Health Center from start to finish. This will include sending referrals to specialists, ensuring their receipt, informing patients and case managers of referrals, and following up to see such appointments are kept
- Request medical records from specialty offices and ensures referring provider reviews are received prior to scanning records into EHR system
- Facilitates transportation for patients as needed through our transportation partner (i.e. Uberhealth, Vivalon)
- Performs assessments of physical, emotional, psychosocial, and environmental needs of the patient to inform the level of care required for each case
- Partner with key individuals in assisting patients in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, medical insurance enrollment, transportation services, or prescription assistance)
- Plan, teach, and coordinate medical care to promote improvement/recovery
- Reinforce patient education presented by primary care and specialty clinicians
- Complete training on HMIS data system and disability processing training
- Work collaboratively with patient advocate, the primary care provider (NP staff) and Transition to Wellness team members to assure coordination of care and benefits
- Proactively work with Chief Medical Officer and Clinic Manager to resolve any technical or flow issues regarding referrals
- Demonstrate appropriate judgment skills to be able to make independent clinical decisions in routine patient care matters
Administrative Assistance for Senior Manager (10%)
- Provide administrative support, to include calendar management (scheduling, prioritizing, etc.), coordination of team meetings, expense management, and support of internal department organization.
- Support senior manager with filing, general paperwork administration, and filling of forms, purchase orders, and submittal of agency requests.
- Work alongside other functional and Executive Admin Assistants to ensure consistency and efficiency.
- Proactively identify process flows which could be improved upon and maintain an active presence in Admin meetings.
- Interact with other departments to conduct special projects and assignments effectively and efficiently.
- Other roles and tasks as assigned.
Qualifications
- Per our Federally Qualified Healthcare Center (FQHC) requirements employment at Ritter Center requires compliance with being able to produce a negative TB test prior to the start of employment, as well as being vaccinated against COVID-19 (unless applying for an exemption) per the August 5th State Public Order for Healthcare Facilities, providers and employees.
- Per the Marin County Department of Health and Human Services, Ritter Center requires all employees to receive the Influenza vaccine (unless applying for a qualified medical or religious exemption)
- Five (5) years of multi-line telephone experience and/or three (3) year experience as medical receptionist with scheduling and telephone experience.
- At least five years of experience in a healthcare setting preferred including experience in private practice, or a Community Health Center.
- Experience working as a patient advocate, culturally competent and able to work with a diverse population and provide top-tier customer service
- Ability to work in a team-oriented environment with staff with different work and communication styles
- Strong problem-solving skills, and ability to be flexible and adaptive to change.
- Understand HMO, PPO, Medicare and Medi-cal insurance plans
- Knowledge of managed care, ICD-9, ICD-10 and CPT codes is preferred
- Excellent organization skills and ability to multitask and juggle multiple priorities
- Ability to remain flexible and responsive when changes occur in patient activity and workload
- Action oriented
- Excellent computer skills; strong proficiency with Microsoft Office
- Resourcefulness in problem solving
- Ability to enter data into various electronic systems while maintaining the integrity and accuracy data
- Excellent verbal, written, and interpersonal skills. Ability to foster an environment for open communication and collaborative practice
- High school diploma combined with medical/clinical experience
- Bilingual in Spanish and English required
