Description
Job Purpose:
The Incident Management Coordinator serves as the UIR Coordinator submits; investigates, reports and monitors incidents, recommends process improvements, staff disciplinary actions and training remediation. This is not a law enforcement position; the purpose of this role is for internal investigations within the Human Services / DDD field.
Essential Functions:
- Serves as UIR Coordinator, including but not limited to, complete initial reports, conduct investigations, complete and review incident investigation/follow-up reports.
- Coordinates with programs to ensure that all requirements pertaining to incident management are accurate and obtained.
- This position acts as a liaison with licensing/funding sources regarding incidents and investigatory process.
- Completes investigations within designated time frames, including follow-up or additional interviews as required.
- Maintains the integrity and fidelity of the investigatory process.
- Prepares investigatory questions based on available information.
- Identifies, reviews, and secures witness statements, documentary, and physical evidence.
- Analyzes and evaluates information and evidence to establish facts and assess credibility.
- Utilizes the required Incident Reporting and Management System (NJIRMS) to monitor incident reports and investigations.
- Documents findings and outlines the facts and analysis, including documenting within the internal tracking system.
- Schedules and participates in meetings with the Administrative Review Team to facilitate a thorough review and discussion of investigation findings and conclusions.
- Develops and distributes corrective action plans for all reportable and internal incidents. Coordinates with appropriate teams to facilitate the implementation of corrective action plans and documents in the designated folders.
- Analyzes investigation trends and makes recommendations to Leadership Team on best practices.
- Monitor and save iRecord plan updates and authorizations to agency EHR.

Requirements
Experience:
- I/DD: 2 years (Required)
- Residential Program or Day Program: 2 years (Required)
- Unusual incident Reporting: 1 year (Preferred)
- Investigation: 1 year (Preferred)
License/Certification:
- New Jersey Driver's License (Required)
