SUMMARY: The Quality and Risk Management Coordinator is responsible for providing data analysis as well as facilitating data collection and presentation to promote quality, safety and performance improvement. Provides support for quality by managing quality initiative teams. Actively involved in the focus on improving safety. Responsible for the Risk Management occurrence database and data analysis. Actively involved in ongoing preparation and response to licensing and accrediting bodies.
ESSENTIAL DUTIES AND RESPONSIBILITIES –include the following. Other duties may be assigned.
Performance improvement and data management
- Facilitate or serve on performance improvement teams, ACA’s, RCA’s and FMEA’s
- Provide analysis of data as well as design studies and data retrieval methods to assist in the promotion of quality, safety and performance improvement.
- Provide coordinating support for maintaining the hospital-wide Quality and Safety program by managing the data collection and the reporting schedule.
- Provide education to leadership and management for quality initiative process, reports, performance improvement plans and PDCA.
- Conduct risk analysis using risk assessment tools and develop tools for quality initiatives such as tracers and audits.
- Actively involved in mandatory data submission on a rotating basis such as Centers for Medicare and Medicaid required LTACH Quality Reporting Program.
- Involved in mandatory reporting for Infection Prevention on an as needed basis for data entry for National Health Safety Network for Infection Prevention.
Culture Of Safety
- Act as a resource to clinical leadership, management staff and staff by providing culture of safety education
- Serve as a safety coach
Serve as the Administrator for the Risk Management data collection system as and provide high level analysis
- Review occurrence reports to confirm level of risk and serious safety event classification.
- Run reports and provide high level analysis.
Actively involved in preparing for and responding to issues identified by external agencies such as The Joint Commission, Department of Public Health, Centers for Medicare and Medicaid and CARF.
- Provides leadership and management updates with any changes from the external agencies.
- Assist in collection of all required documentation and reports
- Provide coordinating support for Regulatory Compliance Committee by assisting with reports, data collection and tracking of audits.

Qualifications:
- Bachelor's degree (B. A.) and two to four years related experience and/or training or equivalent combination of education and experience in healthcare related field. Clinical background if possible. Master’s degree preferred. Quality Control or Quality Assurance experience preferred.
- High level of proficiency with Microsoft Office, including Word, PowerPoint, and Excel. Familiar with and proficient in use of statistical software programs. Ability to maintain confidentiality and use discretion and tact when dealing with managerial and professional staff. Ability to organize and facilitate cross-functional work teams as they examine the underlying causes contributing to non-conforming performance with experience in Root Cause Analysis and/or Failure Mode Effects Analysis a plus. Ability to develop and provide education to staff.
