Lake Forest Place - MDS Coordinator (RN) - Full Time - (8am-4pm) - Monday to Friday - Weekends Call Rotation. 

MDS Coordinator - $81,600-$122,300 Annual Salary (Based on years of experience).

Essential Function:

• Ensure Completion of Resident Assessment Instrument (MDS): Complete MDS assessments within federally mandated guidelines and timelines for each resident.
• Process CATs and CAAs: Handle CATs and CAAs for admissions, 14-day, annual, and significant change MDS assessments. Update resident care plans as needed for both Medicare Part A and non-Medicare residents.
• Evaluate MDS Needs at Readmission: Determine if a new MDS is required upon readmission based on significant, permanent changes in the resident’s condition.
• Notify Involved Disciplines: Communicate key dates to all relevant disciplines, including:
• Admission MDS due dates.
• New MDS initiation and due dates for readmitted residents.
• Significant change MDS due dates.
• Annual MDS due dates.
• Quarterly MDS due dates.
• Medicare-required MDS due dates (e.g., 5-day, 14-day).
• OMRA due dates.
• Maintain MDS Records: Ensure a minimum of 15 months of MDS information is available for each resident in a skilled bed.
• Track MDS Assessments: Keep personal records of admission, Medicare, quarterly, annual, readmission, and significant change reassessment MDS for each resident.
• Assist and Instruct Staff: Guide nursing personnel and other disciplines in the correct MDS completion process.
• Transmit MDS Information: Send MDS data to the Federal Government (CMS) for residents in all certified beds promptly. Maintain transmission logs and validation reports.

Skills/Qualifications:

• Graduation from an approved professional nursing program. BSN or equivalent preferred. 
• Active, current Illinois Registered Nurse license to practice professional nursing.
• Long-term care experience required. 
• Familiarity with MDS and Care Plans recommended. 
• AANAC Certification a plus, if no current certification, expectation that it will be obtained within a year.
