Located in southeast Charlotte near SouthPark and Cotswold, Atrium Health Sardis Oaks is an award-winning nursing and rehabilitation facility with 119 beds. We provide short term rehab, long term care, palliative care, and hospice care and are committed to enhancing the quality of life and well-being of all our residents. LPN teammates at this facility will assist with medication administration, assessments, wound care, bedside care, lab draws, and IV starts.  Sardis Oaks has on-site doctors or nurse practitioners five days a week.  Recently, Sardis Oaks was recognized by Centers for Medicare and Medicaid Services for providing 5 star care and was named one of U.S. News & World Report’s Best Nursing Homes for Short-term Rehabilitation.

Essential Functions

  • Coordinates the assessment, planning, implementation and evaluation for resident care. Ensures that residents' care plans are being implemented.
  • Ensures that unit operation and team members are in compliance with the Nurse Practice Act, regulatory standards, policies and procedures.
  • Organizes and coordinates all nursing unit functions. Identifies staffing needs and schedules teammates appropriately.
  • Manages aspects of team members' compensation and assumes responsibility in recruiting and retaining team members.
  • Promotes team member development through education, performance and identification of goals that enhance professional growth.
  • Performs nursing care, assists with treatment, as well as therapy procedures, and administers medications as necessary.
  • Provides and assesses effectiveness of resident and family education.
  • Participates with performance improvement activities to improve quality of care for the resident.
  • Prepares monthly quality assurance reports as assigned.

Physical Requirements
Must speak English and be able to communicate effectively with residents, visitors and staff. Must have good general vision and hearing, as well as excellent verbal skills and fine motor coordination (finger dexterity), for writing and phone use. Ability to stretch, bend, reach and lift up to 75 pounds in handling residents, medical equipment and supplies. Must be able to respond quickly to changes in the resident and/or unit conditions.



Education, Experience and Certifications

Graduate from an accredited School of Nursing required; Master's of Science in Nursing (MSN) preferred. Enrollment in MSN program within 1 year of hire/transfer date and graduation of MSN program required within 5 years of hire. If undergraduate degree is BSN, a Master's Degree from a health related program or MBA is acceptable. Must have or complete MSN if undergraduate degree is not BSN. Previous nursing supervisory experience required. Appropriate professional leadership certification required (within 2 years if not currently certified) or within 1 year of eligibility for professional certification. BLS required per policy guidelines. Current RN license or temporary license as a Registered Nurse Petitioner required in the state in which you work and reside or; if declaring a National License Compact (NLC) state as your primary state of residency, meet the licensure requirements in your home state; or for Non-National License Compact states, current RN license or temporary license as a Registered Nurse Petitioner required in the state where the RN works.
