Job Summary

The social worker provides the professional social work component of the ambulatory clinical quality services.  Ambulatory Clinical Quality is a collaborative practice model including patients, nurses, social workers, physicians, advanced practice providers (APPs), other caregivers and the community. The Social Worker will integrate care and services of a select high risk patient population across the continuum of care. He/She works collaboratively with patients, families, physicians, and other members of the health care team to achieve the highest quality outcomes with the most effective use of available resources. Among these may be: evaluating patients for indigent assistance such as clinic vouchers, contacting primary care offices to establish outpatient care, handling referrals from the hospital call center, following up on pending studies, contacting the discharged patient and their PCP with these results and coordinating support services as needed for transition back to home, including providing resources, identifying community resources, and supportive care services through a wide variety of services available. The social worker may act as the primary point of contact for pharmacies, for the consulting firm that evaluates medical necessity documentation, and for other clinical entities within the hospital.  In an effort to reduce readmissions, he/she will serve as liaison between the hospitalist department and outpatient physician offices, providing discharge information where needed and possibly facilitating the scheduling of appointments within the Hamilton Health Care System for follow up after hospital discharge.

The social worker assumes responsibility for an interdisciplinary process which assesses, plans, implements, monitors and measures the effectiveness of interventions to meet patients treatment and transitional needs, within their scope. He/She will be an advocate and informational resource for the patients.The social worker will also work with members of the wellness program to coordinate preventive care.  The social worker will facilitates care along a continuum through effective resource coordination. The goals of Case Management include the achievement of optimal health, access to care and appropriate utilization of resources, balanced with the patient's right to self determination. 

Job Qualifications

• Education:  Bachelor’s degree of Social Work required. Masters of social work preferred.
• Licensure:   Exempt from Georgia Licensure
• Experience:  Prefer experience in acute care setting or post-acute inpatient setting. 

Skills:             The Bachelors prepared Social Worker must be a highly motivated, self-directed individual with a demonstrated understanding of acute care case management issues.  The individual will have excellent verbal and written communication skills, strong listening skills, critical thinking and analytical skills, ability to build positive working relationships with the interdisciplinary team, ability to communicate with multiple levels in the organization (e.g. managers, physicians, clinical and support), government agencies and post-acute providers.  He or she must have the ability to obtain and interpret information in terms of the patients’ needs that are age appropriate, and the ability to advocate/negotiate patient/family assessed needs with Durable Medical Equipment / Home Health Care agencies, Nursing Facilities, and/or insurance companies.
