Specialist-Community Health - Full Time

We are searching for individuals who are compassionate, serve with a purpose, strive for excellence, and are dedicated to supporting their team!  Serves as a bridge between the community and the health care, government and social service systems. Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information. Provides informal counseling, support, and follow-up.

• • High School Diploma or equivalent required.
• • 0-3 years of experience required.
• • Requires basic computer skills.
• • Requires knowledge of medical, community, and social resources.
