Effectively managing the care of patients requires aligning the goals and interests of institutional PAC and home-based care providers with the goals of health systems, health plans and physician organizations. This alignment is accomplished through the development and management of high-performing networks of skilled nursing, in-patient rehab, home health and home-care providers to ensure care delivered is cost-effective, high-quality and clinical outcomes are achieved.

As Director, Care Continuum Services, Post Acute Care, you will:

  • Under the direction of the SVP of Care Continuum Services and Market Presidents, develop and implement strategies to support market operational and financial goals.
  • Support the management of all high-performing Post-Acute Networks, primarily focused on skilled nursing facilities, but should also take into consideration existing market relationships and joint ventures with IRFs, LTACHs, home health, and critical access hospitals
  • Review all regularly scheduled reports for accuracy, trends, and indicators for further drill-down to provide status updates on performance of the network and individual network facilities/agencies
  • Collaborate with internal and client analytics teams to request ad-hoc analysis to identify opportunities and strategies for improving the performance of facilities and networks
  • Develop operational report-out presentations to provide current status updates to Navvis internal and partner stakeholders regarding performance status, improvement strategies and obstacles requiring support to address.
  • Support care management operations through clinical and operational expertise, identifying opportunities to optimize value-based performance through strategies such as additional training, workflow evaluation, facility relationship management, etc. 
  • Complete research, independently and as directed, into current government and payer policies, payment models, technological, and regulatory impacts that have the potential to impact performance of high-performing networks
  • Plan, coordinate, develop and deliver high-quality, specialized documents, written reports, and presentations for client organizations
  • Support the sales process through analysis of opportunities, development of materials, and participation in meetings as subject matter expert
Requirements

We are excited about you if you have these things:

  • 5 or more years managing clinical and operational expectations of patient care management in skilled nursing facilities
  • Demonstrated experience in developing comprehensive post-acute strategies
  • Proven track record leading a clinical team and fostering collaboration to maximize patient outcomes and experience
  • Baseline knowledge of regulatory requirements governing skilled nursing/home health, , MSSP/ACOs, Medicare Advantage, CMS Waiver Programs, TEAM, PDPM, etc. 
  • Skill in organizing and prioritizing work, effectively managing multiple projects, delegating, and achieving goals and objectives
  • Skill in exercising a high degree of initiative, professionalism, judgment, and discretion
  • Ability to work effectively independently and as part of a team
  • Strong critical thinking, problem identification and problem resolution skills
  • Superior verbal and written communication skills
  • A focus on detail and accuracy of work
  • Strong business acumen
  • Willingness to travel up to 70%
  • St. Louis-based preferred but not required
