AccordCare is seeking a bilingual Licensed Practical Nurse (LPN) who is committed to delivering compassionate, skilled care in the home setting. • What You Will Do
• As a vital member of our home health team, you will:
• • Provide skilled nursing care tailored to each client’s unique needs
• • Monitor health conditions and document changes in clinical status
• • Administer medications and perform wound care
• • Deliver G-tube, trach, or vent support when required
• • Educate and guide clients and families to support ongoing health at home
• • Communicate effectively with the care team to ensure continuity of care
• Who We Are Looking For
• • Licensed as an LPN and in good standing with the Georgia Board of Nursing
• • Experienced in a clinical setting, with at least one year preferred
• • Certified in CPR
• • Skilled in EMR documentation, with strong organizational ability
• • Confident working independently in a home environment
• • Compassionate, client-focused, and a strong communicator
• • Experienced with G-tube, trach, or vent care
• • Licensed to drive with reliable transportation
